Overview of the medicine
Ondansetron is an anti-emetic drug used to prevent nausea and vomiting caused by chemotherapy, radiation therapy, and surgery.
Uses & Indications
Dosage
Adults
Chemotherapy-induced Nausea and Vomiting (CINV): 8 mg orally 30 minutes before chemotherapy, then 8 mg every 8 hours for up to 1-2 days after chemotherapy. Postoperative Nausea and Vomiting (PONV): 16 mg orally one hour before anesthesia.
Elderly
Similar to adult dosage, but half-life may be prolonged.
Renal_impairment
No dosage adjustment required.
How to Take
May be taken with or without food. Measure the dose carefully using the provided measuring device.
Mechanism of Action
Ondansetron selectively antagonizes 5-HT3 receptors located both peripherally on vagal nerve terminals and centrally in the chemoreceptor trigger zone (CTZ) in the area postrema. This action blocks the initiation of the vomiting reflex.
Pharmacokinetics
Onset
Within 30 minutes for oral administration.
Excretion
Excreted via urine (approx. 5%) and feces.
Half life
Approximately 3-5 hours (adults), 6 hours (elderly).
Absorption
Rapidly and completely absorbed from the GI tract. Peak plasma concentrations are reached in about 1.5 hours.
Metabolism
Primarily metabolized in the liver by hydroxylation, followed by glucuronide or sulfate conjugation.
Side Effects
Contraindications
- Hypersensitivity to ondansetron or any component of the formulation.
- Concomitant use with apomorphine.
Drug Interactions
Tramadol
May reduce the analgesic effect of tramadol.
Apomorphine
Concomitant use is contraindicated due to increased risk of profound hypotension and loss of consciousness.
Phenytoin, Carbamazepine, Rifampicin
May decrease ondansetron plasma levels.
Serotonergic drugs (e.g., SSRIs, SNRIs)
Increased risk of serotonin syndrome.
Storage
Store at room temperature (20-25°C), away from light and moisture. Do not freeze.
Overdose
Symptoms include visual disturbances, severe constipation, hypotension, and vasovagal episodes with transient AV block. Treatment is supportive.
Pregnancy & Lactation
Pregnancy Category B. Use in pregnancy only if clearly needed. Excreted in breast milk, caution advised.
