Description
Overview of the medicine
Ertugliflozin is an SGLT2 inhibitor used to improve glycemic control in adults with type 2 diabetes mellitus and to reduce the risk of major adverse cardiovascular events in certain patients.
Uses & Indications
Dosage
Adults
Initial dose is 5 mg once daily, taken in the morning. May be increased to a maximum of 15 mg once daily if needed and tolerated.
Elderly
No specific dose adjustment is required based solely on age; however, renal function should be assessed before initiation and periodically thereafter.
Renal_impairment
Not recommended for patients with an eGFR <30 mL/min/1.73m². Dosage adjustment may be needed for eGFR 30-<60 mL/min/1.73m².
How to Take
Oral administration, once daily, preferably in the morning, with or without food.
Mechanism of Action
Ertugliflozin inhibits sodium-glucose co-transporter 2 (SGLT2) in the renal tubules, reducing glucose reabsorption and increasing glucose excretion in the urine, thereby lowering blood glucose levels.
Pharmacokinetics
Onset
Glucose-lowering effect typically observed within hours of administration.
Excretion
Approximately 66% excreted in feces and 33% in urine.
Half life
Approximately 11-16 hours.
Absorption
Rapidly absorbed, with peak plasma concentration (Cmax) occurring approximately 1 hour post-dose.
Metabolism
Primarily metabolized via UGT1A9 and UGT2B7 pathways.
Side Effects
Contraindications
- Severe renal impairment, end-stage renal disease, or dialysis.
- Hypersensitivity to ertugliflozin or any component of the formulation.
Drug Interactions
Lithium
SGLT2 inhibitors may decrease serum lithium levels.
Diuretics
May increase the risk of dehydration and hypotension.
Insulin or Insulin Secretagogues
Increased risk of hypoglycemia when used concomitantly; dose adjustment may be necessary.
Storage
Store below 30°C (86°F) in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Overdose management should involve symptomatic and supportive treatment. Ertugliflozin is minimally dialyzable.
Pregnancy & Lactation
Not recommended during the second and third trimesters of pregnancy. Not recommended during breastfeeding; consider discontinuing breastfeeding or the drug.

Clinical Trials
Ertugliflozin has been extensively studied in clinical trials to evaluate its efficacy and safety in glycemic control and cardiovascular outcomes in patients with type 2 diabetes.
Lab Monitoring
- Renal function (eGFR) before initiation and periodically.
- Glycemic control (HbA1c, fasting plasma glucose).
- Lipid profile.
Doctor Notes
- Assess renal function (eGFR) before initiating ertugliflozin and periodically thereafter.
- Counsel patients on the signs and symptoms of dehydration, urinary tract infections, and diabetic ketoacidosis.
- Consider dose adjustment of concomitant insulin or insulin secretagogues to minimize hypoglycemia risk.
Patient Guidelines
- Drink adequate fluids to prevent dehydration.
- Monitor for symptoms of urinary tract or genital infections and seek medical attention if they occur.
- Report any symptoms of ketoacidosis (e.g., nausea, vomiting, abdominal pain, breathlessness, confusion) immediately.
Missed Dose Advice
If a dose is missed, take it as soon as you remember, unless it is almost time for your next dose. Do not take a double dose to make up for a missed one.
Driving Precautions
Ertugliflozin generally does not directly impair driving ability. However, hypoglycemia (especially when used with insulin or sulfonylureas) or symptoms like dizziness may affect concentration and reaction time.
Lifestyle Advice
- Maintain a balanced diet and regular exercise as part of diabetes management.
- Stay well-hydrated.
- Regularly monitor blood glucose levels as advised by your doctor.
