Description
Overview of the medicine
Esonam 40 mg Capsule contains Esomeprazole, a proton pump inhibitor (PPI) that reduces the amount of acid produced in your stomach. It is used to treat acid-related diseases of the stomach and gut, such as gastroesophageal reflux disease (GERD), peptic ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
GERD: 20-40 mg orally once daily for 4-8 weeks. Erosive Esophagitis: 20-40 mg once daily for 4-8 weeks. H. pylori eradication: 40 mg once daily for 10 days (in combination therapy). Zollinger-Ellison Syndrome: Initial dose 40 mg twice daily, adjusted as needed.
Elderly
No dose adjustment generally required, but monitor for adverse effects.
Renal_impairment
No dose adjustment required for patients with renal impairment.
How to Take
Take the capsule orally, usually 30-60 minutes before a meal. Swallow whole with water; do not chew, crush, or open the capsule.
Mechanism of Action
Esomeprazole selectively and irreversibly inhibits the H+/K+-ATPase enzyme (proton pump) in the gastric parietal cells, thereby reducing gastric acid secretion.
Pharmacokinetics
Onset
Acid inhibitory effect onset within 1 hour. Full effect within 3-4 days of therapy.
Excretion
Approximately 80% is excreted in the urine and 20% in the feces as metabolites.
Half life
Plasma elimination half-life is approximately 1-1.5 hours, but acid suppression lasts much longer due to irreversible binding.
Absorption
Rapidly absorbed. Bioavailability is approximately 89% after repeated dosing. Food reduces absorption, so it's taken before meals.
Metabolism
Extensively metabolized in the liver via cytochrome P450 enzymes (primarily CYP2C19 and CYP3A4).
Side Effects
Contraindications
- Hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation
- Co-administration with nelfinavir
Drug Interactions
Warfarin
May cause changes in INR and prothrombin time.
Nelfinavir
Contraindicated due to significant decrease in nelfinavir exposure.
Clopidogrel
May decrease the antiplatelet effect of clopidogrel.
Methotrexate
May increase methotrexate levels, especially with high doses.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
No specific antidote. Management involves symptomatic and supportive treatment. In case of suspected overdose, contact a poison control center or emergency medical services.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if clearly needed. Esomeprazole is excreted in human milk; use with caution during breastfeeding.
