Description
Overview of the medicine
Amlodipine is a calcium channel blocker used to treat high blood pressure (hypertension) and chest pain (angina). It works by relaxing blood vessels, which makes it easier for the heart to pump blood and reduces the workload on the heart.
Uses & Indications
Dosage
Adults
Initial dose 5 mg once daily, maximum 10 mg once daily. For hypertension, usual maintenance dose is 5-10 mg once daily. For angina, 5-10 mg once daily.
Elderly
Initial dose 2.5 mg once daily, gradually increase if needed, considering potential lower clearance.
Renal_impairment
No dosage adjustment needed. Amlodipine is not dialyzable.
How to Take
Take orally once daily, with or without food. It is recommended to take it at the same time each day.
Mechanism of Action
Amlodipine is a dihydropyridine calcium antagonist that inhibits the transmembrane influx of calcium ions into cardiac and vascular smooth muscle. This reduces peripheral vascular resistance and lowers blood pressure. It also dilates coronary arteries and arterioles, increasing myocardial oxygen supply in angina.
Pharmacokinetics
Onset
1-2 hours for antihypertensive effect.
Excretion
Excreted predominantly as inactive metabolites in urine (approx. 60%) and feces (approx. 25%).
Half life
30-50 hours.
Absorption
Well absorbed orally, peak plasma concentrations in 6-12 hours. Bioavailability 64-90%.
Metabolism
Extensive hepatic metabolism to inactive metabolites.
Side Effects
Contraindications
- Known hypersensitivity to dihydropyridines (e.g., amlodipine) or any component of the formulation
- Cardiogenic shock
- Severe aortic stenosis
- Unstable angina or acute myocardial infarction (within the first 28 days)
Drug Interactions
Simvastatin
Amlodipine may increase simvastatin exposure; limit simvastatin dose to 20 mg/day.
Other Antihypertensives
Concurrent use may result in additive hypotensive effects.
CYP3A4 Inducers (e.g., rifampicin, St. John's Wort)
May decrease amlodipine plasma concentrations, potentially reducing efficacy. Dosage adjustment may be needed.
CYP3A4 Inhibitors (e.g., ketoconazole, clarithromycin, ritonavir)
May increase amlodipine plasma concentrations, leading to increased hypotensive effect. Close monitoring is recommended.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include excessive peripheral vasodilation with marked hypotension and reflex tachycardia. Treatment involves symptomatic support, including monitoring of cardiac and respiratory function, elevation of extremities, and administration of vasopressors and intravenous fluids if necessary.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to fetus. Small amounts excreted in breast milk, caution advised. Consult doctor before use during pregnancy or lactation.
