Exiben 100 mg Suspension contains Ibandronic Acid, a bisphosphonate medication used to prevent and treat osteoporosis. It works by slowing down bone loss and increasing bone density, reducing the risk of fractures.
Uses & Indications
Dosage
Adults
For osteoporosis, the typical oral dose of ibandronic acid is 150 mg once a month. For a 100 mg suspension, specific dosing would be determined by a physician. If used daily, 100 mg could be considered. Administer at least 60 minutes before the first food or drink of the day (except plain water).
Elderly
No specific dose adjustment is required for elderly patients based on age alone, but renal function should be considered.
Renal_impairment
For creatinine clearance >30 mL/min, no dosage adjustment is needed. For creatinine clearance ≤30 mL/min, use is generally not recommended or requires careful medical assessment.
How to Take
Take orally, at least 60 minutes before the first food, drink (other than plain water), or other medication of the day. Do not lie down for at least 60 minutes after taking the dose.
Mechanism of Action
Ibandronic acid inhibits osteoclast-mediated bone resorption. It binds to hydroxyapatite crystals in the bone matrix and is taken up by osteoclasts. Inside osteoclasts, it disrupts their normal function, leading to decreased bone breakdown.

Onset
Bone turnover markers begin to decrease within a few days, full effect on BMD takes months.
Excretion
Approximately 50% of the absorbed dose is excreted renally; the unabsorbed portion is excreted in feces.
Half life
Terminal half-life is variable, approximately 10-72 hours, but effectively much longer due to bone binding.
Absorption
Oral absorption is low, typically less than 10%. Absorption is reduced by food and beverages (except plain water).
Metabolism
Not metabolized in humans; excreted unchanged.
Side Effects
Contraindications
- Hypocalcemia
- Inability to stand or sit upright for at least 60 minutes
- Hypersensitivity to ibandronic acid or any excipients
- Severe renal impairment (creatinine clearance ≤ 30 mL/min)
- Esophageal abnormalities which delay esophageal emptying (e.g., stricture or achalasia)
Drug Interactions
NSAIDs
Increased risk of gastrointestinal irritation.
Other Oral Medications
Administer Exiben 100 mg Suspension separately to avoid absorption interference.
Proton Pump Inhibitors (PPIs)
May reduce the efficacy of bisphosphonates (controversial, monitor BMD).
Calcium-containing products & Antacids
May interfere with absorption. Take Exiben 100 mg Suspension at least 60 minutes before.

Overdose
Overdose can lead to hypocalcemia, hypophosphatemia, and upper gastrointestinal adverse events (e.g., esophageal injury, heartburn, esophagitis). Manage symptomatically, provide milk or antacids to bind ibandronic acid.
