Uses & Indications
Dosage
Adults
Initial: 10 mg once daily, may be increased to 20 mg/day after at least one week based on response. For 5 mg tablet, it's typically a starting dose or for maintenance.
Elderly
Initial: 5 mg once daily. Max 10 mg/day.
Renal_impairment
No dosage adjustment needed for mild to moderate impairment. Use with caution in severe impairment.
How to Take
Take orally once daily, with or without food. Swallow the tablet whole with water. Do not crush or chew.
Mechanism of Action
Escitalopram selectively inhibits the reuptake of serotonin (5-HT) in the central nervous system (CNS), leading to an increased concentration of serotonin in the synaptic cleft. This potentiation of serotonergic neurotransmission is thought to be responsible for its antidepressant and anxiolytic effects.

Side Effects
Contraindications
- Hypersensitivity to escitalopram or citalopram
- Concomitant use with MAOIs (Monoamine Oxidase Inhibitors) or within 14 days of stopping an MAOI
- Concomitant use with pimozide
- Congenital long QT syndrome or acquired long QT syndrome
Drug Interactions
MAOIs
Increased risk of serotonin syndrome (hyperthermia, rigidity, myoclonus, autonomic instability).
Alcohol
Enhances CNS depressant effects.
Pimozide
Increased pimozide levels and risk of QT prolongation.
NSAIDs/Aspirin/Warfarin
Increased risk of bleeding.
Serotonergic drugs (e.g., Triptans, Tramadol, St. John's Wort)
Increased risk of serotonin syndrome.
Storage
Store in a cool, dry place below 30°C, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms include dizziness, tremor, agitation, somnolence, coma, nausea, vomiting, tachycardia, hypotension, and QT prolongation. Management involves supportive care, maintaining airway, oxygenation, and ventilation. Gastric lavage and activated charcoal may be considered. No specific antidote.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies the potential risk to the fetus. Not recommended during breastfeeding as escitalopram is excreted in breast milk.
