Description
Overview of the medicine
Fenobate 200 mg capsule is a lipid-lowering medication used to treat high cholesterol and triglyceride levels in the blood, helping to reduce the risk of cardiovascular disease.
Uses & Indications
Dosage
Adults
The usual dose is one 200 mg capsule once daily with a meal.
Elderly
No specific dose adjustment is required in elderly patients with normal renal function; however, renal function should be monitored.
Renal_impairment
Dose adjustment is required for mild to moderate renal impairment. Fenofibrate is contraindicated in severe renal impairment.
How to Take
Take the capsule orally once daily with a main meal to enhance absorption.
Mechanism of Action
Fenofibrate works by activating peroxisome proliferator-activated receptor alpha (PPARα), which increases the synthesis of enzymes that regulate fat metabolism and enhances the elimination of triglyceride-rich particles from the blood.
Pharmacokinetics
Onset
Lipid-lowering effects are generally observed within a few weeks of consistent therapy.
Excretion
Primarily excreted in urine (60-90% as metabolites) and feces (5-25%).
Half life
Approximately 20-23 hours for the active metabolite, fenofibric acid.
Absorption
Well absorbed from the gastrointestinal tract, especially when taken with food. Bioavailability is enhanced by food.
Metabolism
Rapidly hydrolyzed by esterases to its active metabolite, fenofibric acid, which then undergoes glucuronidation.
Side Effects
Contraindications
- Severe renal impairment (eGFR < 30 mL/min)
- Severe hepatic impairment or active liver disease
- Pre-existing gallbladder disease
- Hypersensitivity to fenofibrate or any component of the formulation
- Known photoallergy or phototoxic reaction during treatment with fibrates or ketoprofen
Drug Interactions
Cyclosporine
May impair renal function and increase fenofibrate levels. Close monitoring of renal function is recommended.
Oral Anticoagulants (e.g., Warfarin)
May potentiate the anticoagulant effect, increasing the risk of bleeding. Close monitoring of INR and dose adjustment of anticoagulant may be necessary.
Statins (HMG-CoA Reductase Inhibitors)
Increased risk of myopathy and rhabdomyolysis. Use with caution and monitor for muscle symptoms and creatinine kinase levels.
Bile Acid Sequestrants (e.g., Cholestyramine)
May interfere with fenofibrate absorption. Administer fenofibrate at least 1 hour before or 4-6 hours after bile acid sequestrants.
Storage
Store in a cool, dry place below 25°C, away from direct light and moisture. Keep out of reach of children.
Overdose
There is no specific antidote for fenofibrate overdose. Management should consist of symptomatic and supportive measures. Gastric lavage or activated charcoal may be considered in cases of recent overdose.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not known whether fenofibrate is excreted in human milk; caution should be exercised when administered to a nursing woman.

Clinical Trials
Numerous clinical trials have been conducted to evaluate the efficacy and safety of fenofibrate in improving lipid profiles and reducing cardiovascular events, supporting its role in dyslipidemia management.
Lab Monitoring
- Liver function tests (LFTs) including ALT and AST, periodically during treatment (e.g., every 3 months during the first year, then annually)
- Renal function tests (RFTs) including serum creatinine and eGFR, periodically
- Lipid profile (total cholesterol, LDL-C, HDL-C, triglycerides) to assess treatment efficacy
- Creatine Kinase (CK) levels if muscle pain or weakness occurs
Doctor Notes
- Monitor patient's lipid profile, renal function, and liver function regularly.
- Exercise caution when co-administering with statins or oral anticoagulants.
- Advise patients to report any muscle pain or unexplained tenderness promptly.
Patient Guidelines
- Take the medicine exactly as prescribed by your doctor, usually once a day with food.
- Do not stop taking the medicine without consulting your doctor, even if you feel well.
- Inform your doctor immediately if you experience unexplained muscle pain, tenderness, or weakness, especially with fever.
- Maintain a healthy diet and regular exercise as advised by your doctor to enhance the effectiveness of the medication.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and return to your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Fenobate is generally not expected to affect the ability to drive or operate machinery. However, if you experience dizziness or any other side effects that impair concentration, you should avoid such activities.
Lifestyle Advice
- Adopt a low-fat, low-cholesterol diet.
