Flubac-DS 250 mg Suspension contains Flucloxacillin, a penicillin-group antibiotic used to treat bacterial infections. It is effective against various Gram-positive bacteria, particularly penicillinase-producing staphylococci, commonly causing skin and soft tissue infections, bone and joint infections, and respiratory tract infections.
Uses & Indications
Dosage
Adults
Oral: 250-500 mg every 6 hours, or as directed by the physician. For severe infections, the dose may be increased.
Elderly
No specific dose adjustment is required unless there is significant renal impairment. Monitor renal function.
Renal_impairment
For severe renal impairment (creatinine clearance < 10 mL/min), the dose should be reduced or the dosing interval extended (e.g., 250 mg every 8 or 12 hours) under medical supervision.
How to Take
Flubac-DS suspension should be taken orally. For optimal absorption, it is best to take the medicine 30-60 minutes before meals. Shake the bottle well before each use to ensure uniform dosage. Use the provided measuring cup or spoon for accurate dosing.
Mechanism of Action
Flucloxacillin exerts its bactericidal action by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the cross-linking of peptidoglycan polymers, leading to cell lysis and bacterial death. It is resistant to degradation by beta-lactamase enzymes produced by some bacteria.

Bactericidal effects typically begin within 1-2 hours of administration.
Excretion
Mainly excreted unchanged in the urine (60-70%) via tubular secretion and glomerular filtration. A small amount is excreted in the bile.
Half life
Approximately 0.5 to 1 hour in individuals with normal renal function.
Absorption
Rapidly absorbed from the gastrointestinal tract, especially when taken on an empty stomach. Peak plasma concentrations are achieved within 1-2 hours.
Metabolism
Partially metabolized in the liver, primarily to inactive metabolites.

Contraindications
- Hypersensitivity to flucloxacillin, other penicillins, or any component of the formulation.
- History of flucloxacillin-associated jaundice or hepatic dysfunction.
Drug Interactions
Probenecid
Increases and prolongs plasma levels of flucloxacillin by reducing its renal tubular secretion.
Methotrexate
Flucloxacillin may reduce the renal clearance of methotrexate, potentially increasing its toxicity.
Oral Contraceptives
May reduce the effectiveness of oral contraceptives, advising additional barrier methods.
Warfarin and other oral anticoagulants

Storage

Overdose
In case of an overdose, symptomatic and supportive treatment should be given. Gastric lavage may be considered soon after ingestion. Hemodialysis is not effective in removing flucloxacillin from the blood.

Pregnancy: Flucloxacillin should be used during pregnancy only if clearly needed and after careful consideration of the risks and benefits. Lactation: Flucloxacillin is excreted in breast milk in small amounts. Use with caution in nursing mothers, as it may lead to sensitization or gastrointestinal disturbances in the infant.
