Overview of the medicine
Flukin-500 mg Capsule contains Fluconazole, an antifungal medication used to treat a variety of fungal infections, particularly those caused by Candida species. It is effective for systemic infections, vaginal candidiasis, and other mucosal candidiasis.
Uses & Indications
Dosage
Adults
For vaginal candidiasis: Single oral dose of 150 mg. For cryptococcal meningitis: 400 mg on day 1, followed by 200-400 mg once daily for 6-8 weeks. For other systemic candidiasis: 400 mg on day 1, then 200-400 mg once daily. For some severe or resistant infections, higher doses up to 800 mg daily may be used.
Elderly
Dosage should be adjusted based on renal function.
Renal_impairment
Reduced dose required for creatinine clearance < 50 mL/min. For CrCl 21-50 mL/min, 50% of the usual dose. For CrCl < 20 mL/min, 25% of the usual dose, or administer usual dose every 2 days.

Flukin-500 mg Capsule should be swallowed whole with water, with or without food. For effective treatment, complete the full course of medication as prescribed by the doctor, even if symptoms improve.
Mechanism of Action
Fluconazole inhibits the fungal cytochrome P450 enzyme 14α-demethylase, which is responsible for the demethylation of lanosterol to ergosterol. Ergosterol is an essential component of the fungal cell membrane; its inhibition leads to increased cell membrane permeability and leakage of cellular contents, ultimately causing fungal cell death.

Peak plasma concentrations usually achieved within 1-2 hours after oral administration.

Mainly renal, 80% as unchanged drug.
Half life
Approximately 30 hours (range 20-50 hours).
Absorption
Well absorbed orally (over 90%), bioavailability is not affected by food.
Metabolism
Minimally metabolized; primarily unchanged drug excreted.
Side Effects
Contraindications
- Hypersensitivity to Fluconazole or other azole antifungals
- Co-administration with terfenadine, astemizole, cisapride, pimozide, or quinidine due to risk of QTc prolongation and arrhythmias
Drug Interactions
Rifampin
Decreased fluconazole levels, dose adjustment may be needed.
Warfarin
Increased prothrombin time, monitor INR.
Phenytoin
Increased phenytoin levels, monitor.
Cyclosporine, Tacrolimus, Sirolimus
Increased levels of immunosuppressants, monitor renal function and drug levels.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include hallucinations and paranoid behavior. Treatment involves symptomatic and supportive care, with hemodialysis potentially accelerating clearance of Fluconazole.
Pregnancy & Lactation
Pregnancy Category D (single dose for vaginal candidiasis is Category C). Avoid during pregnancy, especially during the first trimester, unless the benefits outweigh the risks. Fluconazole is excreted in breast milk; caution is advised during breastfeeding.
