Description
Overview of the medicine
Flupen 125 mg Suspension is an antibiotic from the penicillin group, effective against a wide range of bacterial infections. It is particularly useful against penicillinase-producing staphylococci.
Uses & Indications
Dosage
Adults
250-500 mg every 6 hours, usually taken half an hour before meals.
Elderly
No specific dose adjustment is generally required unless renal impairment is present.
Renal_impairment
Dose reduction may be necessary in severe renal impairment (creatinine clearance <10 ml/min). For example, 125-250 mg every 8-12 hours.
How to Take
Shake the bottle well before each use. Administer with the provided measuring spoon or cup. Best taken half an hour before meals on an empty stomach to ensure maximum absorption.
Mechanism of Action
Flucloxacillin inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death. It is resistant to degradation by beta-lactamase enzymes produced by certain bacteria.
Pharmacokinetics
Onset
Rapid (within 1-2 hours)
Excretion
Primarily excreted unchanged by the kidneys via tubular secretion and glomerular filtration. A small amount is excreted in bile.
Half life
Approximately 0.5-1.5 hours in adults with normal renal function.
Absorption
Well absorbed from the gastrointestinal tract, especially when taken on an empty stomach. Peak plasma concentrations are achieved within 1-2 hours.
Metabolism
Partially metabolized in the liver to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to penicillin antibiotics
- History of flucloxacillin-associated jaundice/hepatic dysfunction
- Porphyria
Drug Interactions
Warfarin
May enhance the anticoagulant effect of warfarin, requiring close INR monitoring.
Probenecid
Increases plasma levels and prolongs the half-life of flucloxacillin by reducing renal tubular secretion.
Methotrexate
Increases methotrexate toxicity due to competition for renal tubular secretion.
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children. Once reconstituted, store in a refrigerator (2-8°C) and discard after 7-14 days.
Overdose
Symptoms may include nausea, vomiting, diarrhea, and electrolyte imbalance. Treatment is supportive and symptomatic; gastric lavage or activated charcoal may be considered if recent ingestion. Hemodialysis is not effective.
Pregnancy & Lactation
Pregnancy Category B. Flucloxacillin is generally considered safe during pregnancy. Small amounts are excreted in breast milk; generally considered compatible with breastfeeding, but monitor for rash or diarrhea in the infant.

Lab Monitoring
- Liver function tests (especially with prolonged use or in patients with pre-existing hepatic impairment)
- Renal function tests (in patients with pre-existing renal impairment)
- Complete blood count (rarely, in prolonged high-dose therapy)
Doctor Notes
- Emphasize taking on an empty stomach for optimal absorption.
- Caution patients about completing the full course to prevent antibiotic resistance.
- Monitor liver function tests in patients with pre-existing hepatic disease or prolonged therapy.
Patient Guidelines
- Complete the full course of antibiotics, even if symptoms improve, to prevent resistance.
- Take on an empty stomach, 30-60 minutes before meals.
- Shake the suspension well before each use.
- Do not store leftover medicine; discard after the recommended period (usually 7-14 days after reconstitution).
Missed Dose Advice
If a dose is missed, take it as soon as you remember, unless it is almost time for your next dose. Do not double the dose to catch up.
Driving Precautions
Generally, flucloxacillin does not affect the ability to drive or operate machinery. However, if you experience dizziness or other side effects that impair concentration, avoid these activities.
Lifestyle Advice
- Maintain good hygiene to prevent reinfection.
- Drink plenty of fluids unless advised otherwise by your doctor.
- Avoid alcohol consumption during treatment if experiencing side effects like nausea or dizziness.
