Description
Overview of the medicine
Flutavir (Oseltamivir Phosphate) is an antiviral medication used to treat and prevent influenza (flu) caused by influenza A and B viruses. It works by preventing the flu virus from spreading inside the body.
Uses & Indications
Dosage
Adults
Treatment: 75 mg twice daily for 5 days. Prophylaxis: 75 mg once daily for at least 10 days.
Elderly
No dosage adjustment is generally required for elderly patients with normal renal function.
Renal_impairment
Dosage adjustment is required for patients with creatinine clearance ≤30 mL/min. Consult physician for specific dosage.
How to Take
Take orally, with or without food. Taking with food may improve tolerability in some patients and reduce gastrointestinal side effects. It should be started as soon as possible after the onset of flu symptoms, preferably within 48 hours.
Mechanism of Action
Oseltamivir is a prodrug that is converted to oseltamivir carboxylate, which is a potent and selective inhibitor of influenza A and B virus neuraminidase enzymes. These enzymes are crucial for viral release from infected cells and subsequent spread within the respiratory tract.
Pharmacokinetics
Onset
Within hours of administration, clinical effects typically within 1-2 days.
Excretion
Excreted predominantly in the urine as oseltamivir carboxylate via glomerular filtration and tubular secretion.
Half life
Oseltamivir: 1-3 hours; Oseltamivir carboxylate: 6-10 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract and extensively converted to the active metabolite, oseltamivir carboxylate.
Metabolism
Primarily metabolized by hepatic esterases to its active form, oseltamivir carboxylate. No involvement of cytochrome P450 system.
Side Effects
Contraindications
- Hypersensitivity to oseltamivir phosphate or any component of the formulation.
Drug Interactions
Probenecid
Concomitant administration of probenecid results in approximately a 2-fold increase in exposure to oseltamivir carboxylate due to competition for renal tubular secretion. Dosage adjustment is not required.
Live Attenuated Influenza Vaccine
Oseltamivir may inhibit the replication of the vaccine virus and may decrease the effectiveness of the vaccine. Avoid administration within 2 weeks prior to and for 48 hours after live attenuated influenza vaccine (LAIV).
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children.
Overdose
Limited information is available. Symptoms of overdose may include nausea, vomiting, and in rare cases, neuropsychiatric events. Treatment is symptomatic and supportive. Hemodialysis is not effective in removing oseltamivir carboxylate.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is unknown whether oseltamivir is excreted in human milk. Exercise caution when administered to a nursing woman.
