Description
Overview of the medicine
Fluticon 50 mcg Nasal Spray contains fluticasone propionate, a corticosteroid used to treat and prevent symptoms of allergic rhinitis such as sneezing, runny nose, itchy nose, and watery eyes.
Uses & Indications
Dosage
Adults
Initially, 2 sprays (50 mcg/spray) into each nostril once daily, or 1 spray into each nostril twice daily. Once symptoms are controlled, dose may be reduced to 1 spray into each nostril once daily.
Elderly
No dosage adjustment generally required. Use lowest effective dose.
Renal_impairment
No dosage adjustment required.
How to Take
For intranasal use only. Shake well before each use. Prime the pump if not used for a prolonged period. Tilt head forward slightly, insert nozzle into nostril, and spray while sniffing gently. Avoid spraying into eyes.
Mechanism of Action
Fluticasone propionate is a synthetic trifluorinated corticosteroid with potent anti-inflammatory activity. It acts by binding to glucocorticoid receptors in the nasal mucosa, thereby inhibiting the release of inflammatory mediators (e.g., histamines, prostaglandins, leukotrienes) and reducing the inflammatory response, which leads to a reduction in symptoms of allergic rhinitis.
Pharmacokinetics
Onset
Clinical improvement typically seen within 12-24 hours; maximal effect may take several days.
Excretion
Primarily excreted in the feces, with a small amount excreted in the urine.
Half life
Approximately 7-8 hours (terminal half-life of absorbed drug).
Absorption
Low systemic bioavailability (typically <2%) after intranasal administration due to extensive first-pass metabolism and incomplete absorption from the GI tract of the swallowed portion.
Metabolism
Extensive hepatic metabolism, primarily via CYP3A4 enzyme.
Side Effects
Contraindications
- Hypersensitivity to fluticasone propionate or any excipients in the formulation.
- Untreated localized infection involving the nasal mucosa.
Drug Interactions
Ritonavir
Concomitant use with ritonavir is not recommended due to significantly increased systemic exposure to fluticasone, leading to potential corticosteroid side effects.
Ketoconazole
Co-administration may increase fluticasone systemic exposure, but usually not to a clinically significant extent, although caution is advised.
Storage
Store below 30°C. Do not freeze. Protect from light and moisture. Keep out of reach of children.
Overdose
Acute overdose is unlikely due to the low systemic absorption of intranasally administered fluticasone. Chronic overdose may lead to signs and symptoms of hypercorticism, including adrenal suppression. Management includes gradual withdrawal of the drug and symptomatic treatment.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Small amounts may be excreted in breast milk; caution should be exercised when administered to a nursing mother.

Clinical Trials
Numerous clinical trials have demonstrated the efficacy and safety of fluticasone propionate nasal spray for allergic rhinitis, showing significant reductions in nasal and ocular symptoms compared to placebo. It has been extensively studied in both adult and pediatric populations.
Lab Monitoring
- Regular eye examinations for long-term users (due to potential for glaucoma/cataracts).
- Monitoring for signs of adrenal suppression in patients on high doses or co-administered with strong CYP3A4 inhibitors.
Doctor Notes
- Advise patients on proper administration technique to ensure optimal efficacy and minimize local side effects.
- Counsel on the importance of regular, consistent use rather than intermittent use.
- Monitor for signs of candidiasis and nasal mucosal atrophy with prolonged use.
Patient Guidelines
- Use regularly for best results, even if symptoms improve.
- Do not stop using without consulting your doctor.
- Avoid blowing your nose immediately after use.
- Clean the nozzle regularly.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not double dose to make up for a missed one.
Driving Precautions
Fluticasone nasal spray is not known to affect the ability to drive or operate machinery.
Lifestyle Advice
- Identify and avoid known allergens if possible.
- Keep your living environment clean and free of dust mites.
- Consider using air purifiers if allergic to pollen or dust.
