Dosage

Adults

500 mg intravenously every 12-24 hours, depending on the severity of infection and susceptibility of the pathogen. Infuse over 60-90 minutes.

Elderly

Renal_impairment

For creatinine clearance <30 mL/min, reduce dose to 250 mg every 24 hours.

How to Take

Administer as an intravenous infusion slowly over 60-90 minutes. Do not administer as a rapid bolus injection. Ensure patency of the IV line.
