Description
Overview of the medicine
Furosemide is a potent loop diuretic used to treat edema (fluid retention) associated with congestive heart failure, liver cirrhosis, and renal disease. It is also used to manage hypertension.
Uses & Indications
Dosage
Adults
Edema: Initial 20-80 mg as a single dose, adjusted based on response. Hypertension: 20-40 mg twice daily.
Elderly
Lower initial doses and gradual titration, as they may be more susceptible to adverse effects.
Renal_impairment
Dosage adjustment required. In severe renal impairment, higher doses may be needed, but with caution to avoid ototoxicity.
How to Take
Frusin syrup is for oral administration. It can be taken with or without food. To minimize the chance of nighttime urination, take the last dose several hours before bedtime.
Mechanism of Action
Furosemide acts primarily on the thick ascending limb of the loop of Henle, inhibiting the reabsorption of sodium and chloride ions. This leads to increased excretion of water, sodium, chloride, potassium, and hydrogen ions, resulting in diuresis.

Oral: 30-60 minutes.
Excretion
Mainly by renal excretion (60-80% as unchanged drug) and some biliary excretion.
Half life
1-1.5 hours in healthy individuals, can be prolonged in renal impairment.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract following oral administration. Bioavailability is approximately 50-70%.
Metabolism
Primarily hepatic, with some renal metabolism. It undergoes glucuronidation.
Side Effects
Contraindications
- Anuria (absence of urine production)
- Hepatic coma and pre-coma states
- Severe electrolyte depletion (e.g., severe hypokalemia, hyponatremia)
- Hypersensitivity to furosemide or sulfonamides
Drug Interactions
Digoxin
Increased risk of digoxin toxicity due to furosemide-induced hypokalemia.
Lithium
Furosemide may decrease renal excretion of lithium, leading to increased lithium levels and toxicity.
Antihypertensive agents
Potentiation of antihypertensive effect; dose adjustment may be required.
Aminoglycoside antibiotics
Increased risk of ototoxicity.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May reduce the diuretic and antihypertensive effects of furosemide.

Symptoms of overdose include dehydration, electrolyte imbalance (hypokalemia, hyponatremia, hypochloremia), hypotension, and gastrointestinal disturbances. Management involves symptomatic treatment, fluid and electrolyte replacement, and monitoring of vital signs.
Pregnancy & Lactation
Pregnancy Category C. Furosemide should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in human breast milk; therefore, discretion should be used when administering to nursing mothers. Consider discontinuing the drug or nursing.
