Description
Overview of the medicine
Fusidic Acid and Hydrocortisone Cream is a topical preparation combining an antibacterial agent (Fusidic Acid) and a corticosteroid (Hydrocortisone). It is used to treat skin conditions where bacterial infection is present or likely to occur alongside inflammation, such as eczema or dermatitis.
Uses & Indications
Dosage
Adults
Apply a thin layer to the affected skin area 2-3 times daily. Treatment duration usually for up to two weeks, or as directed by physician. Do not use for more than 7 days unless advised by a doctor.
Elderly
No specific dosage adjustments required for elderly patients, use as per adult dosage.
Renal_impairment
No dosage adjustments required for topical application.
How to Take
For external use only. Wash hands before and after applying the cream. Apply a thin layer gently to the affected area. Avoid contact with eyes, mouth, and other mucous membranes. Do not cover the treated area with occlusive dressings unless advised by a doctor.
Mechanism of Action
Fusidic acid inhibits bacterial protein synthesis by interfering with the translocation step of the elongation factor G (EF-G), thereby preventing the addition of new amino acids to the growing peptide chain. Hydrocortisone is a corticosteroid that reduces inflammation, itching, and redness by suppressing the immune response and inhibiting the release of inflammatory mediators.
Pharmacokinetics
Onset
Anti-inflammatory and antibacterial effects typically begin within a few hours to a day after application.
Excretion
Systemically absorbed fusidic acid is excreted mainly in bile. Systemically absorbed hydrocortisone metabolites are primarily excreted via urine and bile.
Half life
Not directly applicable for topical cream for systemic half-life. Systemic half-life of fusidic acid is ~5-6 hours. Hydrocortisone applied topically has negligible systemic half-life implications.
Absorption
Topical application leads to minimal systemic absorption for both fusidic acid and hydrocortisone, especially when skin is intact. Absorption may increase with broken skin, occlusive dressings, or application to large surface areas.
Metabolism
Fusidic acid is primarily metabolized in the liver, but systemic metabolism is minimal with topical application. Hydrocortisone is metabolized in the liver to inactive metabolites, but again, systemic metabolism is negligible with topical use.
Side Effects
Contraindications
- Hypersensitivity to fusidic acid, hydrocortisone, or any excipients
- Fungal skin infections (e.g., Athlete's foot, ringworm)
- Viral skin infections (e.g., Herpes simplex, chickenpox)
- Tuberculous skin infections
- Perioral dermatitis
- Rosacea
- Acne vulgaris
- Ulcerated skin areas
Drug Interactions
None significant
Systemic absorption is minimal, so significant drug interactions are unlikely with topical use. However, prolonged or extensive use may theoretically lead to some systemic absorption and potential interactions with drugs metabolized by CYP3A4 for hydrocortisone, but this is rare in practice.
Storage
Store below 30°C. Do not freeze. Keep out of reach of children. Protect from light and moisture.
Overdose
Topical overdose is unlikely to cause serious systemic effects due to minimal absorption. Excessive use may lead to local adverse effects such as skin thinning, stretch marks, or increased risk of local infection. If large amounts are ingested orally, seek medical attention.
Pregnancy & Lactation
Use during pregnancy and lactation should only be considered if the potential benefit outweighs the potential risk to the fetus or infant. Avoid extensive use, large areas, or prolonged periods. Consult a doctor before use.

Lab Monitoring
- Not typically required for routine topical use.
Doctor Notes
- Emphasize limited duration of use (usually max 2 weeks).
- Advise against use on viral/fungal infections.
- Caution patients about potential skin thinning with prolonged use, especially on face or in children.
- Consider bacterial resistance if no improvement.
Patient Guidelines
- Complete the full course of treatment even if symptoms improve.
- Do not use for longer than recommended by your doctor.
- Avoid applying to eyes, mouth, or internal body cavities.
- Inform your doctor if your condition does not improve or worsens.
