Description
Overview of the medicine
Diazepam is a benzodiazepine derivative that has anxiolytic, anticonvulsant, sedative, skeletal muscle relaxant, and amnestic properties. It is used to treat anxiety disorders, withdrawal symptoms, muscle spasms, and certain types of seizures. The injection form is used for rapid action in acute conditions.
Uses & Indications
Dosage
Adults
Adults: For acute anxiety/agitation: 2-10 mg IM or IV, repeated in 3-4 hours if necessary. For status epilepticus: 5-10 mg IV, repeated if necessary at 10-15 minute intervals up to max 30 mg. For muscle spasm: 5-10 mg IM or IV, repeated if necessary.
Elderly
Elderly: Initial dose should be reduced to half of the usual adult dose (e.g., 2.5-5 mg IV/IM), with careful titration based on response.
Renal_impairment
Renal impairment: No specific dose adjustment generally required, but caution is advised. Monitor for excessive sedation.
How to Take
For intravenous (IV) administration, inject slowly into a large vein at a rate not exceeding 5 mg/minute to minimize risk of venous irritation and thrombophlebitis. For intramuscular (IM) administration, inject deeply into a large muscle mass.
Mechanism of Action
Diazepam enhances the effect of gamma-aminobutyric acid (GABA) by binding to the benzodiazepine site on the GABA-A receptor, leading to increased chloride ion influx, hyperpolarization, and reduced neuronal excitability.
Pharmacokinetics
Onset
IV: 1-5 minutes for anxiolytic/sedative effects. IM: 15-30 minutes.
Excretion
Primarily excreted in urine as glucuronide conjugates.
Half life
Parent drug: 20-50 hours. Active metabolite (desmethyldiazepam): 30-100 hours.
Absorption
Rapid and complete absorption after intramuscular (IM) injection, but may be erratic. Rapid after intravenous (IV) injection.
Metabolism
Extensively metabolized in the liver via CYP3A4 and CYP2C19 to active metabolites (desmethyldiazepam, temazepam, oxazepam).
Side Effects
Contraindications
- Hypersensitivity to diazepam or other benzodiazepines
- Acute narrow-angle glaucoma
- Severe respiratory insufficiency
- Severe hepatic insufficiency
- Myasthenia gravis
- Sleep apnea syndrome
Drug Interactions
Alcohol
Increased CNS depression.
Opioids
Increased risk of profound sedation, respiratory depression, coma, and death due to additive CNS depressant effects.
Antacids
Delayed absorption but not reduced.
Phenytoin
May alter phenytoin levels.
Antifungals (e.g., ketoconazole)
Increased diazepam levels due to CYP3A4 inhibition.
Storage
Store at room temperature (below 30°C), protect from light. Do not freeze.
Overdose
Symptoms include somnolence, confusion, diminished reflexes, coma. In severe cases, respiratory depression and hypotension. Treatment is supportive, including airway management, IV fluids, and flumazenil (a benzodiazepine antagonist) as an antidote if severe CNS/respiratory depression is present and not due to mixed overdose.
Pregnancy & Lactation
Pregnancy Category D. May cause fetal harm. Avoid during pregnancy, especially in the first trimester. Excreted in breast milk, avoid during breastfeeding or monitor infant for sedation/feeding difficulties.
