Montelukast is a leukotriene receptor antagonist used for the prevention and chronic treatment of asthma and for the relief of symptoms of seasonal and perennial allergic rhinitis. This 5 mg tablet is typically prescribed for pediatric patients aged 6 to 14 years.

One 5 mg tablet taken once daily in the evening. For asthma, it should be taken in the evening. For allergic rhinitis, the time of administration may be individualized to suit patient needs.

One 10 mg tablet taken once daily in the evening (5 mg tablet is not typically indicated for this age group for routine use).
How to Take
Administer orally once daily in the evening. The tablet may be taken with or without food. For allergic rhinitis, the time of administration may be individualized.
Mechanism of Action
Montelukast selectively antagonizes the CysLT1 receptor, blocking the action of leukotriene D4 (LTD4), a potent inflammatory mediator. This action reduces bronchoconstriction and inflammatory responses in the airways, improving asthma control and alleviating allergic rhinitis symptoms.

Excretion
Primarily excreted in the feces (approximately 86%) via bile; less than 0.2% is excreted in urine.

Approximately 2.7 to 5.5 hours.
Absorption
Rapidly absorbed after oral administration; mean oral bioavailability is approximately 64%. Peak plasma concentrations are reached in about 2-4 hours.
Metabolism
Extensively metabolized in the liver by cytochrome P450 enzymes, primarily CYP3A4, 2C8, and 2C9.
Side Effects
Contraindications
- Known hypersensitivity to montelukast or any component of the formulation.
Drug Interactions
Rifampin
Can induce montelukast metabolism, potentially reducing its efficacy. Clinical monitoring is recommended.
Phenobarbital
May decrease the area under the curve (AUC) of montelukast by approximately 40%, requiring clinical monitoring.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In post-marketing experience and clinical studies, there have been reports of acute overdose. The most frequently observed adverse events were abdominal pain, somnolence, thirst, headache, vomiting, and psychomotor hyperactivity. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Montelukast is excreted in breast milk; caution is advised when administered to a nursing woman.
