Description
Overview of the medicine
Gavista 500 mg Suspension contains Gabapentin, an anticonvulsant and neuropathic pain agent. It is used to treat partial seizures and neuropathic pain, such as postherpetic neuralgia.
Uses & Indications
Dosage
Adults
For epilepsy: Initial dose 300 mg three times daily (TID) on Day 1, then titrated up to 900-3600 mg/day in 3 divided doses. For neuropathic pain: Initial 300 mg on Day 1, then titrated up to 1800-3600 mg/day in 3 divided doses.
Elderly
Start with a lower dose (e.g., 100 mg TID) and titrate slowly, monitoring for adverse effects and adjusting for renal function.
Renal_impairment
Dose adjustment required based on creatinine clearance (CrCl). For CrCl < 30 mL/min, significant dose reduction is needed. Hemodialysis patients require supplemental doses.
How to Take
Gavista suspension can be taken with or without food. Measure the dose accurately using a calibrated measuring device provided with the suspension. Do not stop taking it abruptly without consulting your doctor.
Mechanism of Action
Gabapentin's exact mechanism is unknown, but it is thought to modulate voltage-gated calcium channels, specifically binding to the alpha-2-delta subunit, which leads to reduced excitatory neurotransmitter release. It does not bind to GABA receptors directly.
Pharmacokinetics
Onset
Onset of effect varies, typically within days to weeks for full therapeutic benefit.
Excretion
Primarily excreted unchanged by the kidneys. Renal impairment significantly prolongs half-life.
Half life
Approximately 5 to 7 hours in adults with normal renal function.
Absorption
Oral bioavailability is not dose-proportional; it decreases with increasing dose due to saturable absorption.
Metabolism
Not significantly metabolized in humans; it does not induce or inhibit hepatic enzymes.
Side Effects
Contraindications
- Hypersensitivity to Gabapentin or any component of the formulation.
- Acute porphyria.
Drug Interactions
Ethanol/Alcohol
May potentiate the sedative effects of gabapentin.
Opioids (e.g., Morphine)
Concurrent use may increase gabapentin concentrations and increase the risk of CNS depression. Monitor for sedation and respiratory depression.
Antacids (e.g., aluminum and magnesium containing)
May reduce the bioavailability of gabapentin. Administer gabapentin at least 2 hours after antacid administration.
Storage
Store below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include dizziness, somnolence, blurred vision, dysarthria, and diarrhea. Management is primarily supportive; hemodialysis can be used for severe overdose in patients with renal impairment.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Gabapentin is excreted in human milk; caution should be exercised when administered to a nursing mother.

Gabapentin has undergone extensive clinical trials demonstrating its efficacy and safety for the treatment of partial seizures and postherpetic neuralgia. Ongoing research continues to explore its potential in other neuropathic pain conditions.
Lab Monitoring
- Renal function should be monitored, especially in elderly patients or those with pre-existing renal impairment.
Doctor Notes
- Emphasize slow titration to minimize adverse effects, especially dizziness and somnolence.
- Counsel patients on the risk of suicidal ideation and the importance of reporting any behavioral changes.
- Adjust dose carefully in patients with renal impairment and monitor kidney function.
Patient Guidelines
- Do not stop taking Gabapentin abruptly; it should be tapered gradually under medical supervision to avoid withdrawal symptoms.
- Be aware of potential dizziness and drowsiness; avoid driving or operating machinery until you know how the medicine affects you.
- Report any unusual mood changes, thoughts of self-harm, or behavioral changes to your doctor immediately.
Missed Dose Advice
If you miss a dose, take it as soon as you remember, unless it is almost time for your next dose. In that case, skip the missed dose and resume your regular dosing schedule. Do not double the dose to catch up.
Driving Precautions
Gavista may cause dizziness, somnolence, and other central nervous system effects. Patients should be cautioned against driving or operating hazardous machinery until they have sufficient experience to determine whether Gavista adversely affects their ability to perform such tasks.
Lifestyle Advice
- Avoid alcohol consumption while taking Gabapentin as it may increase side effects like dizziness and drowsiness.
- Maintain a healthy lifestyle with regular exercise and a balanced diet.
