Description
Overview of the medicine
Gliclazide 80 mg tablet is an oral antidiabetic medication used to lower blood sugar levels in adults with type 2 diabetes mellitus. It helps the pancreas produce more insulin.
Uses & Indications
Dosage
Adults
Initial dose is 40-80 mg daily, taken with breakfast. The dose can be gradually increased to a maximum of 320 mg daily in divided doses, depending on the patient's response.
Elderly
No specific dose adjustment for elderly patients, but careful monitoring is recommended due to potential increased sensitivity to hypoglycemic effects. Start with a low dose and titrate slowly.
Renal_impairment
In mild to moderate renal impairment, the dose may need to be reduced. Close monitoring of blood glucose and renal function is essential. Contraindicated in severe renal impairment.
How to Take
Glitab 80 mg Tablet should be taken orally once daily with breakfast or the first main meal. It should be swallowed whole with water, not crushed or chewed.
Mechanism of Action
Gliclazide stimulates insulin release from the pancreatic β-cells by blocking ATP-sensitive potassium channels, leading to depolarization and calcium influx, which triggers insulin secretion. It also has some extrapancreatic effects and potential antioxidant properties.
Pharmacokinetics
Onset
Onset of action typically within 1-2 hours.
Excretion
Excreted mainly in the urine (60-70%) and feces (10-20%) as metabolites.
Half life
Plasma half-life of approximately 8-20 hours.
Absorption
Rapidly and completely absorbed from the gastrointestinal tract.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C9, to inactive metabolites.
Side Effects
Contraindications
- Type 1 Diabetes Mellitus
- Diabetic ketoacidosis, diabetic precoma and coma
- Severe renal or hepatic insufficiency
- Hypersensitivity to gliclazide or other sulfonylureas or any excipients of the product
- Pregnancy and lactation
- Concomitant treatment with miconazole
Drug Interactions
Alcohol
Increases the hypoglycaemic reaction, which can lead to hypoglycaemic coma.
Phenylbutazone (systemic route)
Increases the hypoglycaemic effect of sulfonylureas, displacing them from protein binding sites and/or reducing their elimination.
Miconazole (systemic route, oral gel)
Increases the hypoglycaemic effect with possible onset of hypoglycaemia or even coma.
Beta-blockers, ACE inhibitors, Fluconazole, H2-receptor antagonists
May potentiate the blood glucose lowering effect, increasing the risk of hypoglycemia.
Storage
Store in a cool and dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
An overdose of Gliclazide can lead to severe hypoglycemia, which may manifest as confusion, tremor, sweating, hunger, blurred vision, and potentially coma. Treatment involves immediate administration of glucose (oral if conscious, intravenous if unconscious) and careful monitoring.
Pregnancy & Lactation
Gliclazide is contraindicated during pregnancy and lactation. Insulin is the preferred treatment for diabetes during pregnancy. Breastfeeding is not recommended while taking Gliclazide.

Lab Monitoring
- Regular monitoring of blood glucose levels (fasting, postprandial)
- Glycated hemoglobin (HbA1c) every 3-6 months
- Renal function tests (e.g., serum creatinine, eGFR)
- Liver function tests (e.g., ALT, AST)
Doctor Notes
- Emphasize patient education regarding hypoglycemia symptoms and management.
- Monitor renal and hepatic function, especially in elderly patients.
- Consider lifestyle modifications (diet, exercise) as foundational therapy for Type 2 Diabetes Mellitus.
- Avoid concomitant use with miconazole due to severe interaction risk.
Patient Guidelines
- Take the tablet at the same time each day with a meal, preferably breakfast, to reduce the risk of hypoglycemia.
- Do not skip meals and maintain a regular eating pattern.
- Recognize and manage symptoms of hypoglycemia (e.g., sweating, trembling, dizziness) by consuming a quick source of sugar.
- Regularly monitor blood sugar levels as advised by your doctor.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. However, if it's almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not double the dose to make up for a missed one.
Driving Precautions
Patients should be aware of the symptoms of hypoglycemia and exercise caution while driving or operating machinery, especially at the beginning of treatment or after dose adjustments. If hypoglycemic symptoms occur, avoid driving.
Lifestyle Advice
- Adhere to a balanced diet recommended for diabetes, limiting sugar and refined carbohydrates.
- Engage in regular physical activity as advised by your healthcare provider.
- Maintain a healthy weight.
- Avoid or limit alcohol consumption.
