Description
Overview of the medicine
Hemocal-LD 4-2 Rectal Ointment is a combination medication containing Lidocaine Hydrochloride and Hydrocortisone Acetate. It is used for the symptomatic relief of pain, itching, swelling, and discomfort associated with hemorrhoids and other anorectal disorders.
Uses & Indications
Dosage
Adults
Apply a small amount to the affected area 2 to 3 times daily. For internal hemorrhoids, use the applicator and insert a small amount into the rectum 2 to 3 times daily, preferably after bowel movements.
Elderly
Similar to adult dosage, but use with caution and under medical advice, especially if other medical conditions are present.
Renal_impairment
No specific dosage adjustment is typically required for topical application due to minimal systemic absorption.
How to Take
For external or intrarectal use only. Wash and dry the affected area thoroughly before application. Apply a thin layer of ointment. For intrarectal application, attach the provided applicator to the tube, insert gently into the rectum, and squeeze. Wash hands thoroughly before and after use.
Mechanism of Action
Lidocaine, a local anesthetic, blocks nerve impulse conduction, thereby providing temporary pain relief and reducing itching. Hydrocortisone, a corticosteroid, acts as an anti-inflammatory agent, reducing swelling and irritation by suppressing the immune response in the affected area.
Pharmacokinetics
Onset
Local anesthetic effect typically within 5-10 minutes.
Excretion
Metabolites are primarily excreted via urine.
Half life
Systemic half-life for Lidocaine is approximately 1.5-2 hours, and for Hydrocortisone, it's about 1-2 hours. However, systemic absorption from rectal application is generally low.
Absorption
Minimal systemic absorption occurs when applied topically to intact skin or mucous membranes. Absorption can increase with inflamed, abraded, or broken skin/mucosa.
Metabolism
Lidocaine is primarily metabolized in the liver. Hydrocortisone is metabolized locally in the skin and liver.
Side Effects
Contraindications
- Hypersensitivity to Lidocaine, Hydrocortisone, or any other components of the ointment.
- Infections (bacterial, viral, fungal) in the treatment area without concomitant anti-infective therapy.
- Children under 12 years of age without medical supervision.
Drug Interactions
Other Local Anesthetics
Systemic interactions are unlikely due to minimal absorption. However, caution should be exercised if other lidocaine-containing products or local anesthetics are used concurrently in large amounts or over prolonged periods, as systemic effects could potentially accumulate.
Corticosteroids (systemic)
Rarely, if significant systemic absorption of hydrocortisone occurs, it could contribute to systemic corticosteroid effects, especially in patients already on systemic steroids.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach and sight of children.
Overdose
Systemic overdose is highly unlikely with proper topical use. Excessive application may lead to local irritation, burning, or stinging. If significant systemic absorption occurs (e.g., from prolonged use over large, broken areas), symptoms might include central nervous system effects (dizziness, drowsiness, nervousness) or cardiovascular effects (bradycardia, hypotension). In case of accidental ingestion or suspected systemic overdose, seek immediate medical attention.
Pregnancy & Lactation
Use Hemocal-LD with caution during pregnancy and lactation. It should only be used if the potential benefit outweighs the potential risk to the fetus or infant, and under strict medical supervision. Consult a doctor before use.
