Infecef 500 mg Injection contains Cefuroxime, a second-generation cephalosporin antibiotic. It is used to treat a wide range of bacterial infections.
Uses & Indications
Dosage
Adults
750 mg to 1.5 g IV/IM every 8 hours. For severe infections or infections caused by less susceptible organisms, 1.5 g IV every 8 hours.
Elderly
No specific dosage adjustment needed unless there is significant renal impairment. Monitor renal function.
Renal_impairment
Dosage should be reduced based on creatinine clearance (e.g., for CrCl 10-20 mL/min, 750 mg every 12 hours; for CrCl <10 mL/min, 750 mg every 24 hours).
How to Take
Administer by deep intramuscular (IM) injection or slow intravenous (IV) injection over 3 to 5 minutes, or by intravenous infusion over 30 minutes. Reconstitute the powder with sterile water for injection as per manufacturer's instructions.
Mechanism of Action
Cefuroxime works by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the cross-linking of peptidoglycan chains, leading to cell lysis and bacterial death.

Onset
Peak plasma concentrations are achieved within 30-60 minutes after IM/IV administration.
Excretion
Primarily excreted unchanged by the kidneys, through both glomerular filtration and tubular secretion.
Half life
Approximately 1.2 to 1.6 hours.
Absorption
Rapidly absorbed after intramuscular (IM) injection; completely absorbed after intravenous (IV) administration.
Metabolism
Not significantly metabolized in the body.
Side Effects
Contraindications
- Known hypersensitivity to cefuroxime, any other cephalosporin antibiotic, or to any of the excipients.
- History of severe hypersensitivity (e.g., anaphylactic reaction) to any other type of beta-lactam antibacterial agent (e.g., penicillins).
Drug Interactions
Probenecid
Concomitant use with probenecid prolongs the half-life of cefuroxime, resulting in increased plasma concentrations.
Aminoglycosides
Concurrent administration with aminoglycosides may increase the risk of nephrotoxicity.
Oral Anticoagulants
Cefuroxime may affect gut flora, leading to lower vitamin K levels and potentially enhancing the anticoagulant effect of warfarin. INR monitoring is recommended.
Oral Contraceptives
Reduced efficacy of oral contraceptives has been reported. Consider alternative birth control methods.
Storage
Store below 30°C. Protect from light and moisture. Keep out of reach of children. Do not freeze the powder or the reconstituted solution.
Overdose
Overdosage can lead to neurological sequelae including encephalopathy, convulsions, and coma. Management is symptomatic and supportive. Hemodialysis may be effective in removing cefuroxime from the body.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus. Cefuroxime is excreted in small quantities in breast milk; caution should be exercised when administered to a nursing mother. Consult a doctor before use during pregnancy or lactation.
