Description
Overview of the medicine
Inpen 500 mg Injection contains Meropenem, a broad-spectrum carbapenem antibiotic used to treat a wide range of serious bacterial infections, including pneumonia, urinary tract infections, intra-abdominal infections, skin and soft tissue infections, and meningitis.
Uses & Indications
Dosage
Adults
Usual dose is 500 mg to 1 g intravenously every 8 hours, depending on the type and severity of infection.
Elderly
Dose adjustment may be necessary based on renal function.
Renal_impairment
Dose reduction required; e.g., CrCl 26-50 mL/min: 1 adult unit every 12 hours; CrCl 10-25 mL/min: 1/2 adult unit every 12 hours; CrCl <10 mL/min: 1/2 adult unit every 24 hours.
How to Take
For intravenous use only. Administer as an intravenous bolus over 3-5 minutes or as an intravenous infusion over 15-30 minutes. Reconstitution instructions must be followed carefully.
Mechanism of Action
Meropenem is a bactericidal antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Rapid, typically within minutes of IV administration.
Excretion
Primarily excreted unchanged in the urine (approximately 70% within 12 hours).
Half life
Approximately 1 hour in adults with normal renal function.
Absorption
Rapid and almost complete absorption after intravenous administration.
Metabolism
Minimal hepatic metabolism; primarily hydrolyzed by dehydropeptidase-I (DHP-I) in the kidneys, but Meropenem is stable to DHP-I.
Side Effects
Contraindications
- Known hypersensitivity to meropenem, carbapenem antibiotics, or any component of the formulation.
- Patients with a history of severe hypersensitivity reactions to other beta-lactam antibiotics (e.g., penicillins, cephalosporins).
Drug Interactions
Probenecid
Probenecid competes with meropenem for active tubular secretion, leading to increased plasma concentrations and prolonged half-life of meropenem.
Valproic acid
Concomitant use with Meropenem may result in significant reduction in valproic acid plasma concentrations, increasing the risk of seizure.
Oral anticoagulants (e.g., Warfarin)
Increased anticoagulant effect has been reported when carbapenems are co-administered with warfarin, leading to an increased risk of bleeding.
Storage
Store dry powder below 25°C (77°F), protected from light. Reconstituted solution should be used immediately or stored as per manufacturer's guidelines.
Overdose
In the event of an overdose, discontinue Meropenem and initiate general supportive treatment. Hemodialysis can remove meropenem and its metabolite from the body.
Pregnancy & Lactation
Pregnancy Category B. Use in pregnancy only if clearly needed. Meropenem is excreted in human milk; caution should be exercised when administered to a nursing woman.

Lab Monitoring
- Monitor renal function (creatinine, BUN) regularly, especially in patients with pre-existing renal impairment.
- Monitor liver function tests (ALT, AST, ALP) periodically.
- Complete blood count (CBC) should be monitored during prolonged therapy.
Doctor Notes
- Meropenem is a critical antibiotic for treating severe, multidrug-resistant bacterial infections.
- Careful consideration of renal function is essential for dose adjustment, especially in elderly and renally impaired patients.
- Monitor for seizure activity, particularly in patients with CNS disorders or compromised renal function.
Patient Guidelines
- Complete the full course of treatment, even if symptoms improve, to prevent recurrence and development of antibiotic resistance.
- Report any severe side effects immediately to your healthcare provider.
- Inform your doctor about all other medications you are taking.
Missed Dose Advice
If a dose is missed, administer it as soon as remembered. If it is almost time for the next dose, skip the missed dose and resume the regular dosing schedule. Do not double the dose.
Driving Precautions
This medicine may cause dizziness or seizures in some patients. Patients should be advised to exercise caution when driving or operating machinery.
Lifestyle Advice
- Maintain good hydration during treatment.
- Avoid alcohol consumption while on Meropenem.
- Practice good hygiene to prevent further infections.
