Description
Overview of the medicine
Insulet-50-50-injection is a pre-mixed insulin formulation containing 50% insulin lispro protamine suspension and 50% insulin lispro injection. It is used to improve glycemic control in patients with diabetes mellitus.
Uses & Indications
Dosage
Adults
Individualized dosage based on metabolic needs, blood glucose monitoring, and glycemic control goals. Typically administered subcutaneously, 15 minutes before or immediately after a meal.
Elderly
Close monitoring of blood glucose required; dosage adjustments may be necessary due to potential renal/hepatic impairment and increased sensitivity to insulin.
Renal_impairment
Dosage may need to be reduced in patients with renal impairment due to decreased insulin clearance.
How to Take
Administer subcutaneously in the abdominal wall, thigh, upper arm, or buttock. Rotate injection sites to reduce the risk of lipodystrophy. Do not administer intravenously or intramuscularly.
Mechanism of Action
Insulin, including insulin lispro, lowers blood glucose by stimulating peripheral glucose uptake by skeletal muscle and fat, and by inhibiting hepatic glucose production. Insulin inhibits lipolysis and proteolysis, and enhances protein synthesis.
Pharmacokinetics
Onset
15-30 minutes (due to lispro component)
Excretion
Excreted primarily by the kidneys.
Half life
Variable, depends on dose and injection site. Functional half-life for glucose lowering effect can be longer.
Absorption
Rapid absorption of lispro, slower absorption of protamine suspension from subcutaneous tissue.
Metabolism
Metabolized in the liver and kidneys.
Side Effects
Contraindications
- During episodes of hypoglycemia.
- In patients with hypersensitivity to insulin lispro or any of the excipients.
Drug Interactions
Beta-blockers
May mask the symptoms of hypoglycemia.
Oral Antidiabetic Agents
May require dosage adjustments of both agents.
ACE Inhibitors, Salicylates, Sulfonamides
May enhance the hypoglycemic effect of insulin.
Thiazide Diuretics, Corticosteroids, Thyroid Hormones
May decrease the hypoglycemic effect of insulin.
Storage
Store unopened vials/cartridges in a refrigerator (2°C-8°C / 36°F-46°F). Do not freeze. Protect from light. Once opened or in-use, store at room temperature (below 30°C / 86°F) for up to 28 days.
Overdose
Overdose may lead to severe hypoglycemia, which can be treated by oral glucose or glucagon injection. Severe cases may require intravenous glucose.
Pregnancy & Lactation
Insulin requirements may change during pregnancy and lactation. Close monitoring and dosage adjustments are essential. Generally considered safe for use during pregnancy, but consult a healthcare provider.

Doctor Notes
- Emphasize the importance of consistent meal timing and carbohydrate counting.
- Educate patients on proper injection technique and rotation of sites to prevent lipodystrophy.
- Counsel patients on recognizing and treating hypoglycemia.
Patient Guidelines
- Always check the insulin label before each injection.
- Learn proper injection technique and rotate injection sites.
- Monitor blood glucose levels regularly as advised by your doctor.
- Be aware of symptoms of hypoglycemia and how to treat it.
Missed Dose Advice
If a dose is missed, blood glucose should be checked, and a healthcare professional should be consulted. Do not take a double dose to compensate for a missed dose.
Driving Precautions
Patients should be advised to take precautions to avoid hypoglycemia while driving, especially if they experience recurrent hypoglycemia or have reduced or absent warning symptoms of hypoglycemia.
