Description
Overview of the medicine
L-Sol 2 mg Tablet contains Levosulpiride, which is primarily used to treat functional dyspepsia, irritable bowel syndrome (IBS) with predominant dyspepsia, and anxiety or depression with somatic symptoms. It acts as a prokinetic agent and also has mild antipsychotic properties.
Uses & Indications
Dosage
Adults
The usual recommended dose is 25 mg three times daily. For L-Sol 2 mg, a lower dose might be prescribed for specific conditions, typically 2 mg three times daily, or as directed by the physician.
Elderly
A lower initial dose may be considered due to potential reduced renal function. Dosage should be individualized based on response and tolerability.
Renal_impairment
Dosage adjustment is necessary. For mild to moderate impairment, the dose may be reduced to half. For severe impairment, it may be contraindicated or require significant reduction and careful monitoring.
How to Take
Take the tablet orally with water, with or without food. It is generally recommended to take it before meals for better prokinetic effect.
Mechanism of Action
Levosulpiride is a selective antagonist of peripheral and central D2-dopaminergic receptors. Its prokinetic action is primarily due to antagonism of pre-synaptic D2 receptors in the enteric nervous system, leading to increased acetylcholine release and enhanced gastrointestinal motility. At higher doses, it also blocks post-synaptic D2 receptors in the limbic system, contributing to its mood-modulating and anxiolytic effects.
Pharmacokinetics
Onset
Clinical effects on gastric motility typically begin within 30-60 minutes.
Excretion
Mainly excreted by the kidneys (approximately 90% as unchanged drug) within 24 hours.
Half life
Approximately 6-8 hours, with some inter-individual variability.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations reached within 1-3 hours.
Metabolism
Undergoes minimal hepatic metabolism. Excretion primarily occurs unchanged.
Side Effects
Contraindications
- Hypersensitivity to Levosulpiride or any component of the formulation.
- Pheochromocytoma.
- Epilepsy or manic states.
- Prolactin-dependent tumors (e.g., pituitary prolactinoma, breast cancer).
- Gastrointestinal hemorrhage, mechanical obstruction, or perforation where stimulation of gastrointestinal motility could be harmful.
Drug Interactions
Levodopa
Levosulpiride may antagonize the effects of levodopa.
CNS Depressants
Concurrent use may enhance CNS depression (e.g., drowsiness, sedation).
Anticholinergics
May antagonize the prokinetic effect of Levosulpiride.
Drugs prolonging QT interval
Caution advised as Levosulpiride may prolong QT interval (though rare at low doses).
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, sedation, extrapyramidal symptoms, and hypotension. Treatment is symptomatic and supportive, including gastric lavage and activated charcoal if appropriate. ECG monitoring may be necessary.
Pregnancy & Lactation
Not recommended during pregnancy, especially during the first trimester, unless clearly necessary and potential benefits outweigh the risks. Avoid during lactation as Levosulpiride is excreted in breast milk.

Lab Monitoring
- Prolactin levels (especially with long-term use or endocrine symptoms)
- Renal function (in patients with kidney impairment)
Doctor Notes
- Consider baseline prolactin levels before initiating long-term therapy.
- Educate patients about potential endocrine side effects and advise them to report any changes.
- Monitor for extrapyramidal symptoms, particularly in sensitive patients or with dose escalation.
- Use with caution in patients with a history of cardiovascular disease due to potential QT prolongation risk.
Patient Guidelines
- Do not exceed the prescribed dose without consulting your doctor.
- Inform your doctor about all other medications, including over-the-counter drugs and herbal supplements.
- Avoid consumption of alcohol while taking this medicine as it may increase drowsiness.
- If you experience unusual symptoms, contact your physician immediately.
