Description
Overview of the medicine
Lacticon 10 gm powder contains Lactulose, an osmotic laxative used to treat constipation and hepatic encephalopathy by increasing water content and softening stools, or by reducing ammonia absorption.
Uses & Indications
Dosage
Adults
For constipation: 10-20 gm daily as a single dose or in two divided doses. For hepatic encephalopathy: 20-30 gm three to four times daily, adjusted to achieve 2-3 soft stools per day.
Elderly
Same as adult dosage, but may require careful titration.
Renal_impairment
No specific dose adjustment needed as it is minimally absorbed and renally excreted.
How to Take
Dissolve the contents of one sachet in half a glass of water and drink immediately. Can be taken with or without food. Ensure adequate fluid intake.
Mechanism of Action
Lactulose is metabolized by colonic bacteria into low molecular weight organic acids, which increase the osmotic pressure in the colon, drawing water into the lumen. This softens the stool and increases its bulk, promoting peristalsis. In hepatic encephalopathy, it lowers colonic pH, which favors the conversion of ammonia (NH3) to ammonium (NH4+), which is then trapped in the colon and excreted.
Pharmacokinetics
Onset
24-48 hours for laxative effect; 24 hours to several days for hepatic encephalopathy
Excretion
Excreted unchanged in feces; absorbed portion excreted in urine.
Half life
Undefined for systemic circulation due to minimal absorption; largely dependent on colonic transit time.
Absorption
Minimally absorbed (less than 3%)
Metabolism
Not metabolized by human enzymes; metabolized by colonic bacteria.
Side Effects
Contraindications
- Galactosemia
- Intestinal obstruction
- Perforation of the gastrointestinal tract
- Hypersensitivity to lactulose
Drug Interactions
Antacids
May inhibit the action of lactulose by altering colonic pH.
Other laxatives
Concurrent use with other laxatives may increase the risk of electrolyte imbalance.
Oral anti-infectives (e.g., neomycin)
May reduce the degradation of lactulose by colonic bacteria, diminishing its effect.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include abdominal pain, diarrhea, and electrolyte disturbances (especially hypokalemia). Treatment involves symptomatic and supportive care, including fluid and electrolyte replacement.
Pregnancy & Lactation
Generally considered safe during pregnancy and lactation as it is minimally absorbed. Consult a doctor before use.
