Description
Overview of the medicine
Marvelouse-Plus is a combination medication containing Aceclofenac, a non-steroidal anti-inflammatory drug (NSAID), and Thiocolchicoside, a muscle relaxant. It is used to relieve pain and inflammation associated with various musculoskeletal conditions, along with muscle spasms.
Uses & Indications
Dosage
Adults
One tablet twice daily after meals, or as directed by the physician. Duration of treatment should not exceed 7 days.
Elderly
No specific dose adjustment required, but caution is advised due to potential renal/hepatic impairment and concomitant medications.
Renal_impairment
Reduced dosage may be necessary in mild to moderate renal impairment. Avoid in severe renal impairment (CrCl < 30 mL/min).
How to Take
Oral administration. Take the tablet whole with water, preferably after meals to minimize gastrointestinal discomfort.
Mechanism of Action
Aceclofenac works by inhibiting cyclooxygenase (COX) enzymes, leading to reduced prostaglandin synthesis, thereby exerting anti-inflammatory, analgesic, and antipyretic effects. Thiocolchicoside acts as a centrally acting muscle relaxant by selectively binding to GABA-A receptors and glycine receptors, producing its myorelaxant effects without sedation.
Pharmacokinetics
Onset
Pain relief within 30-60 minutes.
Excretion
Aceclofenac metabolites are primarily excreted via urine (approx. 2/3) and feces (approx. 1/3). Thiocolchicoside metabolites are mainly excreted renally.
Half life
Aceclofenac: Approximately 4 hours. Thiocolchicoside: Approximately 2.5 to 5 hours.
Absorption
Aceclofenac is rapidly and completely absorbed from the GI tract, reaching peak plasma concentrations in 1.25 to 3 hours. Thiocolchicoside is also well absorbed orally, with peak plasma concentrations occurring within 1-2 hours.
Metabolism
Aceclofenac is extensively metabolized in the liver, primarily by CYP2C9. Thiocolchicoside is metabolized into an active metabolite (3-desmethylthiocolchicoside).
Side Effects
Contraindications
- Hypersensitivity to Aceclofenac, Thiocolchicoside, or any excipients
- Active peptic ulcer or gastrointestinal bleeding
- History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs
- Severe heart failure, renal failure, or hepatic failure
- Pregnancy and lactation
Drug Interactions
Lithium
Aceclofenac may increase plasma concentrations of Lithium, requiring dose adjustment.
Warfarin
Increased risk of bleeding with anticoagulants like Warfarin due to Aceclofenac.
Diuretics
Reduced natriuretic effect of diuretics by Aceclofenac.
Methotrexate
Increased toxicity of Methotrexate with Aceclofenac.
Corticosteroids
Increased risk of gastrointestinal ulceration or bleeding with concomitant use of corticosteroids.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of Aceclofenac overdose may include headache, nausea, vomiting, epigastric pain, GI bleeding, rarely diarrhea, disorientation, excitation, coma, drowsiness, dizziness, tinnitus, fainting, and convulsions. Thiocolchicoside overdose symptoms are not well established but may include gastrointestinal disturbances. Management involves supportive and symptomatic treatment. Gastric lavage and activated charcoal may be considered within an hour of ingestion.
Pregnancy & Lactation
Contraindicated during pregnancy, especially in the third trimester, due to potential fetal harm (ductus arteriosus closure, renal dysfunction). Avoid during lactation as components may pass into breast milk.
