Description
Overview of the medicine
Mebendox is an anthelmintic agent used to treat various parasitic worm infections such as pinworm, roundworm, whipworm, and hookworm.
Uses & Indications
Dosage
Adults
Pinworm: 100 mg as a single dose, repeated in 2-4 weeks. Other infections (roundworm, whipworm, hookworm): 100 mg twice daily for 3 consecutive days.
Elderly
No specific dose adjustment is generally required for elderly patients.
Renal_impairment
No specific dose adjustment is needed for patients with renal impairment, as absorption is minimal and excretion is primarily fecal.
How to Take
Take orally with or without food. The suspension can be swallowed directly. For pinworm, it is advisable to treat all household members simultaneously.
Mechanism of Action
Mebendazole selectively and irreversibly blocks glucose uptake and other nutrients in susceptible intestinal nematodes. This leads to the depletion of glycogen stores, reduced ATP formation, and ultimately the death of the parasite.
Pharmacokinetics
Onset
Therapeutic effects usually seen within a few days of starting treatment.
Excretion
Primarily excreted via feces (unabsorbed drug) and urine (metabolites).
Half life
Approximately 2.5 to 5.5 hours in plasma.
Absorption
Poorly absorbed systemically from the gastrointestinal tract (about 5-10%). Absorption is increased when taken with a fatty meal.
Metabolism
Rapidly and extensively metabolized in the liver, primarily by CYP450 enzymes, to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to mebendazole or any components of the formulation.
- Pregnancy (especially first trimester, unless clearly needed and benefits outweigh risks).
- Children under 2 years of age for mass treatment (though commonly used for therapeutic reasons under medical supervision).
Drug Interactions
Cimetidine
May inhibit the metabolism of mebendazole, leading to increased plasma concentrations.
Metronidazole
Concurrent use has been associated with an increased risk of Stevens-Johnson syndrome/toxic epidermal necrolysis (rare cases).
Phenytoin, Carbamazepine
May decrease mebendazole plasma levels due to enzyme induction.
Storage
Store below 30°C. Protect from light and moisture. Keep out of reach of children. Do not freeze.
Overdose
In case of overdose, symptoms may include abdominal cramps, nausea, vomiting, and diarrhea. Treatment is symptomatic and supportive, including gastric lavage if ingested recently.
Pregnancy & Lactation
Pregnancy Category C. Consult a doctor before use. Small amounts are excreted into breast milk, generally considered compatible with breastfeeding at usual doses, but physician consultation is advised.
