Description
Overview of the medicine
Moxibest 400 mg Tablet contains Moxifloxacin, a broad-spectrum fluoroquinolone antibiotic used to treat a variety of bacterial infections.
Uses & Indications
Dosage
Adults
400 mg once daily for 5-14 days, depending on the infection.
Elderly
No dosage adjustment is generally required for elderly patients.
Renal_impairment
No dosage adjustment is required for patients with mild to severe renal impairment (creatinine clearance ≤30 mL/min) or end-stage renal disease on hemodialysis or continuous ambulatory peritoneal dialysis (CAPD).
How to Take
Moxibest 400 mg tablet should be swallowed whole with a glass of water, with or without food. Do not chew or crush the tablet.
Mechanism of Action
Moxifloxacin inhibits bacterial DNA gyrase and topoisomerase IV, essential enzymes involved in bacterial DNA replication, transcription, repair, and recombination, leading to bacterial cell death.
Pharmacokinetics
Onset
Within hours of administration, therapeutic concentrations reached at infection sites.
Excretion
Approximately 45% of the dose is excreted in urine (20% as unchanged drug, 25% as metabolites) and 50% in feces (20% as unchanged drug, 30% as metabolites).
Half life
Approximately 11-15 hours.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract; absolute bioavailability is approximately 90%. Peak plasma concentrations are achieved within 0.5-4 hours.
Metabolism
Undergoes hepatic metabolism mainly via glucuronide and sulfate conjugation, without significant involvement of cytochrome P450 system.
Side Effects
Contraindications
- Hypersensitivity to moxifloxacin or other quinolones.
- History of tendinitis or tendon rupture associated with quinolone use.
- Patients with known prolongation of the QT interval, uncorrected hypokalemia, or significant bradycardia.
- Co-administration with other drugs that prolong the QT interval.
- Severe hepatic impairment.
Drug Interactions
Warfarin
Enhanced anticoagulant effect. INR monitoring is recommended.
Corticosteroids
Increased risk of tendinitis and tendon rupture.
Antacids containing aluminum, magnesium, iron, sucralfate, zinc
May reduce the absorption of moxifloxacin. Administer moxifloxacin several hours before or after these agents.
Drugs that prolong the QT interval (e.g., Class IA and III antiarrhythmics, tricyclic antidepressants, macrolides, antipsychotics)
Increased risk of ventricular arrhythmias, including Torsades de Pointes. Co-administration is contraindicated.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In the event of an acute overdose, patients should be carefully observed and given supportive treatment. Gastric lavage and activated charcoal may be considered shortly after ingestion. ECG monitoring is recommended due to the potential for QT interval prolongation. Hemodialysis or peritoneal dialysis is not expected to be beneficial.
Pregnancy & Lactation
Pregnancy Category C. Moxifloxacin should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted into breast milk. A decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.

Lab Monitoring
- Monitor renal function, especially in patients with pre-existing kidney disease, although dose adjustment is not typically required.
- Monitor liver function tests periodically in patients with hepatic impairment or those developing signs/symptoms of liver injury.
- Monitor ECG for QT interval prolongation in patients at risk.
Doctor Notes
- Counsel patients on the black box warnings, especially regarding tendon issues and neurological adverse events.
- Carefully assess patient history for QT prolongation risk factors before prescribing.
- Avoid co-administration with other QT-prolonging drugs.
- Consider alternative antibiotics for uncomplicated infections when possible.
Patient Guidelines
- Complete the full course of medication even if symptoms improve.
- Do not share your medication with others.
- Inform your doctor if you experience any unusual pain or swelling in tendons.
- Avoid excessive exposure to direct sunlight or artificial UV light.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose to catch up.
