Description
Overview of the medicine
Moxin contains Moxifloxacin, a broad-spectrum fluoroquinolone antibiotic used to treat various bacterial infections including respiratory tract, skin, and abdominal infections.
Uses & Indications
Dosage
Adults
400 mg once daily for the recommended duration depending on the infection type.
Elderly
No dose adjustment is generally required for elderly patients.
Renal_impairment
No dose adjustment is required for patients with mild to severe renal impairment (creatinine clearance ≤30 mL/min/1.73m²), or for patients on hemodialysis or continuous ambulatory peritoneal dialysis.
How to Take
Oral tablets can be taken with or without food. IV infusion should be administered slowly over 60 minutes.
Mechanism of Action
Moxifloxacin inhibits bacterial DNA gyrase and topoisomerase IV, enzymes essential for DNA replication, transcription, repair, and recombination, leading to bacterial cell death.
Pharmacokinetics
Onset
Relatively rapid, peak plasma concentrations typically occur within 0.5 to 4 hours.
Excretion
Excreted in urine (approx. 20%) and feces (approx. 25%) as unchanged drug, and metabolites are also excreted via these routes.
Half life
Approximately 12 hours.
Absorption
Rapidly and almost completely absorbed after oral administration; bioavailability is approximately 90%.
Metabolism
Primarily metabolized in the liver via glucuronide and sulfate conjugation, without cytochrome P450 involvement.
Side Effects
Contraindications
- •Hypersensitivity to moxifloxacin or other quinolones
- •History of QTc prolongation
- •Uncorrected hypokalemia
- •History of tendinitis or tendon rupture associated with fluoroquinolone use
- •Myasthenia gravis
Drug Interactions
Warfarin
May enhance the anticoagulant effect of warfarin. Monitor INR closely.
Corticosteroids
Increased risk of tendinitis and tendon rupture when co-administered with moxifloxacin.
Antiarrhythmics (Class IA and III)
Potential for additive QTc prolongation. Avoid co-administration.
Antacids containing aluminum or magnesium, sucralfate, iron supplements, multivitamin preparations containing zinc
May reduce absorption of moxifloxacin. Administer moxifloxacin several hours before or after these agents.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
In the event of an acute overdose, induce vomiting or perform gastric lavage. Observe the patient and ensure adequate hydration. ECG monitoring is recommended due to potential for QTc prolongation. No specific antidote is available. Symptomatic and supportive treatment should be initiated.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in breast milk; therefore, avoid use during lactation or discontinue breastfeeding.
