Olanap 5 mg Tablet contains Olanzapine, an atypical antipsychotic medicine used to treat conditions such as schizophrenia and bipolar disorder. It helps to restore the balance of certain natural substances in the brain, improving mood, thoughts, and behavior.
Uses & Indications
Dosage
Adults
Schizophrenia: Initial dose is 10 mg once daily. Dosage range 5-20 mg/day. Bipolar Mania: Initial dose is 10-15 mg once daily as monotherapy, or 10 mg once daily as adjunctive therapy. Dosage range 5-20 mg/day.
Elderly
A lower initial dose (e.g., 5 mg/day) may be considered, especially for those with factors that might result in slower metabolism (e.g., non-smoking female patients ≥65 years of age). Close monitoring is recommended.
Renal_impairment
No routine dosage adjustment is required in patients with renal impairment, but caution is advised, particularly in severe cases. Close monitoring of clinical response is recommended.
How to Take
Olanap 5 mg Tablet should be taken orally, once daily, with or without food. It is generally recommended to take it at the same time each day, preferably in the evening, as it can cause drowsiness.
Mechanism of Action
Olanzapine acts as an antagonist at multiple receptor sites, including dopamine D2 and D1, D3, D4 receptors, and serotonin 5-HT2A, 5-HT2C, 5-HT3, 5-HT6 receptors. It also has antagonistic activity at muscarinic M1-M5, histamine H1, and alpha-1 adrenergic receptors. Its antipsychotic effect is primarily attributed to its combined antagonism of dopamine D2 and serotonin 5-HT2A receptors.

Onset
Antipsychotic effects may begin within a few days, but full therapeutic effect may take several weeks. Sedative effects can occur shortly after administration.
Excretion
Approximately 57% of the dose is excreted in the urine and about 30% in feces, mainly as metabolites.
Half life
The elimination half-life ranges from 21 to 54 hours (mean 33 hours).
Absorption
Well absorbed orally; peak plasma concentrations reached in approximately 5-8 hours. Bioavailability is not affected by food.
Metabolism
Extensively metabolized in the liver, primarily via direct glucuronidation and cytochrome P450-mediated oxidation (CYP1A2 and CYP2D6).

Contraindications
- Known hypersensitivity to olanzapine or any component of the formulation
- Patients at high risk of developing narrow-angle glaucoma
- Patients with a history of paralytic ileus

Overdose
Symptoms of overdose typically include drowsiness, tachycardia, extrapyramidal symptoms, blurred vision, respiratory depression, and hypotension. There is no specific antidote. Treatment is supportive and symptomatic, including maintaining an adequate airway, oxygenation, and ventilation, and monitoring cardiovascular function.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Olanzapine is excreted in human milk; therefore, women should not breastfeed while taking this medication.
