Description
Overview of the medicine
Plator-Plus is a combination medicine containing Clopidogrel and Aspirin, used to prevent blood clots in conditions like acute coronary syndrome, heart attack, and stroke.
Uses & Indications
Dosage
Adults
Usually one tablet (Clopidogrel 75 mg + Aspirin 75 mg) once daily.
Elderly
No specific dose adjustment is required for elderly patients.
Renal_impairment
Use with caution in patients with severe renal impairment; dose adjustment may be necessary based on clinical judgment.
How to Take
Oral administration, with or without food. Swallow the tablet whole with water.
Mechanism of Action
Clopidogrel selectively inhibits the binding of adenosine diphosphate (ADP) to its platelet P2Y12 receptor, preventing platelet activation and aggregation. Aspirin irreversibly inhibits cyclooxygenase (COX-1) in platelets, which reduces thromboxane A2 (TXA2) production, further inhibiting platelet aggregation.
Pharmacokinetics
Onset
Within 2-3 hours for antiplatelet effect.
Excretion
Mainly renal excretion for metabolites; some fecal excretion for clopidogrel.
Half life
Clopidogrel: ~6 hours (active metabolite); Aspirin: 15-20 minutes (plasma), antiplatelet effect lasts for the lifetime of platelets (7-10 days).
Absorption
Clopidogrel is rapidly absorbed and extensively metabolized. Aspirin is rapidly and completely absorbed.
Metabolism
Both are extensively metabolized in the liver.
Side Effects
Contraindications
- Active pathological bleeding (e.g., peptic ulcer, intracranial hemorrhage).
- Severe hepatic impairment.
- Known hypersensitivity to clopidogrel, aspirin, or any component of the product.
Drug Interactions
SSRIs
Increased risk of bleeding.
NSAIDs
Increased risk of gastrointestinal bleeding.
Proton Pump Inhibitors (PPIs)
May reduce the antiplatelet effect of clopidogrel (e.g., omeprazole, esomeprazole).
Anticoagulants (e.g., Warfarin)
Increased risk of bleeding.
Storage
Store below 25°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Overdose may lead to increased bleeding risk, epigastric pain, nausea, vomiting, dizziness, tinnitus, and hearing impairment. Management is symptomatic and supportive, with close monitoring for bleeding complications.
Pregnancy & Lactation
Not recommended during pregnancy, especially in the third trimester, due to potential fetal harm (e.g., premature closure of ductus arteriosus). Avoid during lactation as both drugs are excreted in breast milk.
