Description
Overview of the medicine
Povidone-iodine is a broad-spectrum antiseptic used for topical application. As a 10% vaginal gel, it is specifically formulated for treating and preventing various vaginal infections and as an antiseptic for gynecological procedures.
Uses & Indications
Dosage
Adults
Insert one applicator full (approximately 5 grams) intravaginally, preferably at bedtime, once daily for 7 to 14 days, or as directed by a physician.
Elderly
No specific dosage adjustment is generally required. Use with caution if underlying thyroid or renal conditions exist.
Renal_impairment
Use with caution due to potential for systemic iodine absorption. Consult a physician for advice, especially with prolonged or extensive use.
How to Take
For intravaginal use only. Wash hands thoroughly before and after use. The gel should be inserted deeply into the vagina using the provided applicator, preferably at bedtime to allow maximum contact time.
Mechanism of Action
Povidone-iodine releases free iodine, which rapidly penetrates microbial cell walls and oxidizes proteins, nucleotides, and fatty acids, leading to cellular dysfunction and death. It demonstrates broad-spectrum microbicidal activity against bacteria, fungi, viruses, spores, and protozoa.
Pharmacokinetics
Onset
Rapid, typically within minutes of application.
Excretion
Primarily renal for any systemically absorbed iodine and its metabolites.
Half life
Not well-defined for topical application. Systemically absorbed iodine has a renal half-life of 2-5 days.
Absorption
Minimal systemic absorption through intact vaginal mucosa. Absorption can be higher with damaged mucosa, inflammation, or prolonged/extensive use. Absorbed iodine is excreted via kidneys.
Metabolism
Absorbed iodine is metabolized in the body, primarily in the liver, and can be incorporated into thyroid hormones.
Side Effects
Contraindications
- Known hypersensitivity to iodine or povidone
- Thyroid disorders (e.g., hyperthyroidism, goiter, Hashimoto's thyroiditis) due to potential for systemic iodine absorption and interference with thyroid function
- Patients undergoing lithium therapy
- Pregnancy (especially after 2nd trimester) and lactation due to risk of fetal/neonatal hypothyroidism
- Prior to or during radioiodine therapy
Drug Interactions
Lithium
Concomitant use may increase the risk of hypothyroidism due to additive effects on thyroid function.
Octenidine
Avoid concomitant use as it may lead to transient skin discoloration and adverse interactions.
Hydrogen Peroxide, Silver Sulfadiazine, Taurolidine
These substances can interact with povidone-iodine, potentially reducing its antiseptic efficacy.
Storage
Store in a cool, dry place below 30°C, protected from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Excessive vaginal application or accidental ingestion may lead to systemic iodine toxicity. Symptoms can include gastrointestinal distress (nausea, vomiting, diarrhea), abdominal pain, metabolic acidosis, renal impairment, and thyroid dysfunction (hypo- or hyperthyroidism). Treatment is symptomatic and supportive, including gastric lavage if ingested, and monitoring of fluid/electrolyte balance and thyroid function.
Pregnancy & Lactation
Contraindicated in pregnancy (especially after the 2nd trimester) and during lactation due to the potential for significant systemic iodine absorption and subsequent risk of fetal/neonatal thyroid dysfunction (hypothyroidism). Consult a doctor immediately if pregnant or breastfeeding.

Lab Monitoring
- Thyroid function tests (TSH, T3, T4) should be considered for patients on prolonged or extensive povidone-iodine therapy, especially in those with pre-existing thyroid conditions or renal impairment.
- Renal function tests if systemic absorption is a concern.
Doctor Notes
- Educate patients on the proper technique for intravaginal application and the importance of completing the full course of treatment.
- Advise patients about potential staining of clothing and recommend using panty liners.
- Counsel pregnant or lactating women against using this product due to potential risks to fetal/neonatal thyroid function.
- Monitor thyroid function in patients with pre-existing thyroid conditions or those requiring prolonged or extensive therapy.
