Description
Overview of the medicine
Povisep 5% w/w powder is a topical antiseptic containing povidone-iodine, used for preventing and treating skin infections in minor cuts, wounds, burns, and abrasions. It helps to keep the wound clean and promotes healing.
Uses & Indications
Dosage
Adults
Clean the affected area thoroughly. Apply a thin layer of Povisep 5% w/w powder once or twice daily, or as directed by a physician. Cover with a sterile dressing if necessary.
Elderly
No specific dosage adjustment required. Use with caution in elderly patients with pre-existing thyroid conditions.
Renal_impairment
Use with caution in patients with severe renal impairment, especially with extensive application, due to potential for systemic iodine absorption.
How to Take
For external use only. Clean and dry the affected area before applying the powder. Sprinkle a thin layer directly onto the wound or skin infection. Do not ingest. Avoid contact with eyes, ears, and mucous membranes. Wash hands before and after use.
Mechanism of Action
Povidone-iodine works by slowly releasing iodine, which penetrates microbial cell walls and causes oxidative damage to their proteins, nucleotides, and fatty acids. This results in rapid killing of bacteria, viruses, fungi, and protozoa.
Pharmacokinetics
Onset
Rapid onset of action, typically within 30 seconds to a few minutes.
Excretion
Mainly excreted via the kidneys. Some excretion also occurs through feces and sweat.
Half life
Not directly applicable for topical use in terms of systemic half-life for therapeutic effect. Systemically absorbed iodine is eliminated renally with a variable half-life.
Absorption
Minimal systemic absorption when applied topically to intact skin. Absorption may increase through broken skin, mucous membranes, or extensive burns. Absorbed iodine is rapidly distributed in the body.
Metabolism
Absorbed iodine is primarily metabolized in the liver and thyroid gland.
Side Effects
Contraindications
- Known hypersensitivity to iodine or povidone
- Thyroid disorders (e.g., hyperthyroidism, goiter)
- Prior to and after radioiodine therapy
- Newborns and premature infants (due to risk of systemic iodine absorption and thyroid suppression)
- Extensive, deep, or chronic wounds without medical supervision
- Concurrent use with mercury-containing products
Drug Interactions
Lithium
Concurrent use may lead to additive hypothyroid effects, especially in patients with pre-existing thyroid conditions.
Octenidine
Avoid concurrent use as it may lead to transient skin discoloration.
Hydrogen Peroxide
May inactivate povidone-iodine, reducing its antiseptic effect.
Silver Sulfadiazine
May reduce the efficacy of both agents when used concurrently on wounds.
Storage
Store in a cool, dry place below 30°C. Protect from direct sunlight and moisture. Keep the container tightly closed.
Overdose
Accidental ingestion or excessive absorption from large or broken skin areas may lead to systemic iodine toxicity symptoms such as nausea, vomiting, diarrhea, abdominal pain, metabolic acidosis, renal impairment, and thyroid dysfunction (hyper- or hypothyroidism). Management is symptomatic and supportive, including gastric lavage if ingested and monitoring thyroid and renal function.
Pregnancy & Lactation
Use with caution during pregnancy and lactation. Avoid prolonged or extensive application, especially on broken skin, due to potential for systemic iodine absorption and adverse effects on fetal/infant thyroid function. Consult a physician before use.
