Description
Overview of the medicine
Povisep-6-w-cream contains Povidone-iodine, a broad-spectrum antiseptic used for topical disinfection of skin, wounds, and burns to prevent infection.
Uses & Indications
Dosage
Adults
Clean the affected area and apply a thin layer of cream 1-3 times daily, or as directed by a healthcare professional. May be covered with a sterile dressing.
Elderly
No specific dosage adjustment is required for elderly patients. Use as per adult dosage.
Renal_impairment
For topical application, no specific dosage adjustment is necessary. However, caution is advised with extensive application in patients with severe renal impairment due to potential systemic iodine absorption.
How to Take
For external use only. Wash hands before and after application. Apply a small amount of cream directly to the clean, dry affected skin area. Avoid contact with eyes, ears, and mouth.
Mechanism of Action
Povidone-iodine releases free iodine, which rapidly oxidizes microbial proteins and nucleic acids, leading to irreversible cell damage and death of bacteria, fungi, viruses, protozoa, and spores.
Pharmacokinetics
Onset
Rapid onset of action, typically within seconds to minutes of application.
Excretion
Mainly excreted renally as iodide. Small amounts may be excreted in feces, saliva, and sweat.
Half life
Systemic half-life of absorbed iodine is approximately 1-2 days, but topical action is immediate and sustained.
Absorption
Minimal systemic absorption occurs through intact skin. Increased absorption can occur from open wounds, mucous membranes, or extensive burn areas.
Metabolism
Absorbed iodine is primarily metabolized in the liver to iodide, which then enters the general iodine pool.
Side Effects
Contraindications
- Known hypersensitivity to povidone-iodine or any other ingredient
- Thyroid disorders, particularly hyperthyroidism (overactive thyroid)
- Before and after radioiodine therapy
- In neonates and very low birth weight infants due to risk of systemic iodine absorption and thyroid effects
- During pregnancy and lactation on large areas or for prolonged periods
Drug Interactions
Lithium
Concurrent use may enhance the hypothyroid effect of lithium, especially with extensive application.
Octenidine
Use with octenidine-containing products can cause dark discoloration of skin and should be avoided.
Enzymatic wound cleaners
Povidone-iodine may inactivate enzymatic components in some wound cleansers, reducing their efficacy.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Systemic iodine toxicity is rare with topical use but can occur with extensive application to large areas of broken skin or mucous membranes. Symptoms may include metabolic acidosis, renal failure, and thyroid dysfunction. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Use with caution and only if clearly needed. Extensive or prolonged use, especially during the third trimester, may lead to systemic iodine absorption and potential adverse effects on fetal/neonatal thyroid function. Small amounts may be excreted in breast milk; consult a doctor before use during lactation.

Lab Monitoring
- Thyroid function tests (TFTs) may be considered for prolonged or extensive application, especially in patients with pre-existing thyroid conditions or renal impairment.
- Renal function monitoring in cases of suspected significant systemic absorption.
Doctor Notes
- Emphasize the importance of patient education regarding appropriate application, duration of use, and signs of adverse reactions.
- Be cautious when prescribing for patients with pre-existing thyroid conditions, especially hyperthyroidism, or severe renal impairment due to potential systemic effects.
- Advise against concurrent use with other topical agents without consulting a physician, particularly enzymatic debriding agents or octenidine.
Patient Guidelines
- For external use only; do not ingest.
- Avoid contact with eyes, ears, and mouth. Rinse thoroughly with water if contact occurs.
- Discontinue use and consult a doctor if severe irritation, redness, swelling, or signs of an allergic reaction develop.
- Do not use on large areas of broken skin or for prolonged periods without medical advice.
Missed Dose Advice
If a dose is missed, apply it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular schedule. Do not apply extra to make up for a missed dose.
