Description
Overview of the medicine
Povisep-75-W Surgical Scrub is an antiseptic solution containing povidone-iodine, used for skin preparation before surgery and for general skin disinfection.
Uses & Indications
Dosage
Adults
For surgical hand scrub: Wet hands and forearms with water. Apply 5 ml of scrub and spread thoroughly. Scrub for at least 5 minutes. Rinse thoroughly. Repeat if necessary. For pre-operative skin preparation: Apply undiluted to the operative site, allow to dry for at least 2 minutes. May be repeated.
Elderly
Same as adult dosage, but use with caution in individuals with pre-existing thyroid conditions.
Renal_impairment
No specific dosage adjustment required for topical application unless severe renal impairment is accompanied by extensive absorption.
How to Take
For external use only. Apply to the affected area or as directed for surgical preparation. Do not swallow. Avoid contact with eyes, ears, and mucous membranes. If contact occurs, rinse thoroughly with water.
Mechanism of Action
Povidone-iodine releases free iodine when in contact with skin or mucous membranes. This free iodine acts as a broad-spectrum microbicide, rapidly killing bacteria, fungi, viruses, protozoa, and spores by oxidizing cellular components.
Pharmacokinetics
Onset
Rapid microbicidal action, typically within 15-30 seconds.
Excretion
Mainly excreted renally in the form of iodide.
Half life
Not clinically significant for topical application under normal circumstances.
Absorption
Minimal systemic absorption occurs through intact skin. Significant absorption may occur through extensive broken skin or mucous membranes.
Metabolism
Absorbed iodine is metabolized in the liver.
Side Effects
Contraindications
- Known hypersensitivity to iodine or povidone.
- Thyroid disorders (e.g., hyperthyroidism, goitre), especially in neonates and young children.
- Extensive burns, particularly in children.
- Before and after radioiodine therapy.
Drug Interactions
Lithium
May cause additive thyroid effects if systemic absorption of iodine occurs.
Other antiseptics
Do not use with other antiseptics containing octenidine, hydrogen peroxide, or silver sulfadiazine, as they may reduce efficacy or cause adverse reactions.
Mercury-containing compounds
Concomitant use may form corrosive compounds, avoid combination.
Storage
Store below 30°C in a cool, dry place. Protect from light. Do not freeze. Keep out of reach of children.
Overdose
Systemic overdose from topical application is rare but possible with extensive use on damaged skin. Symptoms may include gastrointestinal distress, metabolic acidosis, and renal impairment. Management is supportive and symptomatic.
Pregnancy & Lactation
Use with caution. Extensive or prolonged use, especially on large surface areas, may lead to iodine absorption and potential effects on fetal or infant thyroid function. Consult a healthcare professional before use.

Lab Monitoring
- Thyroid function tests (for prolonged or extensive use, particularly in neonates or patients with thyroid conditions).
- Renal function tests (in cases of significant systemic absorption, e.g., severe burns).
Doctor Notes
- Ensure sufficient contact time for effective antisepsis, typically 2-5 minutes for surgical scrub.
- Advise patients about potential skin irritation and to discontinue if severe reaction occurs.
- Exercise caution in patients with pre-existing thyroid conditions or renal impairment if extensive exposure is anticipated.
Patient Guidelines
- For external use only. Do not ingest.
- Avoid contact with eyes. If contact occurs, rinse immediately with plenty of water.
- Do not apply to extensive areas of broken skin or deep wounds without medical advice.
- Discontinue use and consult a doctor if irritation or allergic reaction occurs.
