Description
Overview of the medicine
Prazopress 1 mg Tablet is an alpha-1 adrenoceptor blocker used to treat high blood pressure and symptoms of benign prostatic hyperplasia (BPH). It works by relaxing blood vessels and the muscles in the prostate and bladder.
Uses & Indications
Dosage
Adults
Initial dose 0.5 mg two or three times daily. Gradually increase to 1 mg two or three times daily. Usual maintenance dose is 3-20 mg daily in divided doses. Max: 20 mg/day for hypertension, 10 mg/day for BPH.
Elderly
Start with lowest dose (0.5 mg) and titrate carefully.
Renal_impairment
No specific dose adjustment necessary for mild to moderate impairment, but careful monitoring is advised.
How to Take
Prazopress tablet should be taken orally, with or without food. It is often recommended to take the first dose at bedtime to minimize the risk of 'first-dose phenomenon' (sudden drop in blood pressure).
Mechanism of Action
Prazosin selectively blocks postsynaptic alpha-1 adrenoceptors, resulting in peripheral vasodilation (relaxation of blood vessels). This reduces peripheral vascular resistance and lowers blood pressure. In BPH, it relaxes smooth muscle in the prostate and bladder neck, improving urine flow.
Pharmacokinetics
Onset
Blood pressure lowering effect typically begins within 30-60 minutes.
Excretion
Primarily excreted in the feces via bile, with a small portion excreted in urine.
Half life
Approximately 2-3 hours.
Absorption
Well absorbed from the gastrointestinal tract. Peak plasma concentrations are reached within 1-3 hours.
Metabolism
Extensively metabolized in the liver, primarily by demethylation and conjugation.
Side Effects
Contraindications
- Hypersensitivity to prazosin or other quinazolines
- History of orthostatic hypotension
Drug Interactions
NSAIDs
May reduce the antihypertensive effect.
PDE5 Inhibitors (e.g., Sildenafil)
Risk of symptomatic hypotension.
Beta-blockers, Diuretics, other Antihypertensives
Increased hypotensive effect.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms include severe hypotension, dizziness, drowsiness, and syncope. Treatment involves supporting cardiovascular function (e.g., elevation of legs, fluid administration, vasopressors if needed).
Pregnancy & Lactation
Pregnancy Category C. Use only if clearly needed and the potential benefits outweigh the risks. Prazosin is excreted in small amounts in breast milk; use with caution in breastfeeding mothers.

Lab Monitoring
- Regular blood pressure monitoring
- Monitoring for signs of orthostatic hypotension
Doctor Notes
- Counsel patients on the 'first-dose phenomenon' and ways to mitigate it.
- Advise caution when co-administering with PDE5 inhibitors due to increased risk of hypotension.
- Monitor blood pressure regularly, especially during dose titration.
Patient Guidelines
- Take the first dose at bedtime to minimize dizziness.
- Avoid sudden changes in posture to prevent orthostatic hypotension.
- Report any prolonged erections immediately.
- Do not stop taking the medicine abruptly without consulting your doctor.
Missed Dose Advice
If you miss a dose, take it as soon as you remember, unless it is almost time for your next dose. In that case, skip the missed dose and resume your regular dosing schedule. Do not double the dose.
