Description
Overview of the medicine
Cefixime is an oral third-generation cephalosporin antibiotic used to treat various bacterial infections susceptible to cefixime.
Uses & Indications
Dosage
Adults
The usual adult dose is 200 mg once daily or 100 mg twice daily. For severe infections or infections caused by less susceptible organisms, the dose may be increased to 400 mg once daily.
Elderly
No dosage adjustment is necessary for elderly patients unless they have severe renal impairment.
Renal_impairment
For creatinine clearance 21-60 mL/min, the dose should be reduced to 75% of the standard dose. For creatinine clearance <20 mL/min, the dose should be reduced to 50% of the standard dose.
How to Take
Supraxim-200-mg-capsule should be taken orally. It can be taken with or without food. Taking it with food may help reduce gastrointestinal upset.
Mechanism of Action
Cefixime acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the final transpeptidation step of peptidoglycan synthesis, leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved within 2 to 6 hours after a single 200 mg dose.
Excretion
Approximately 50% of the absorbed dose is excreted unchanged in the urine over 24 hours. Non-renal clearance accounts for about 10% of total clearance.
Half life
Plasma half-life is typically 3 to 4 hours, which may be prolonged in patients with renal impairment.
Absorption
Approximately 40-50% orally absorbed. Food may delay absorption but does not significantly alter the extent of absorption.
Metabolism
Cefixime undergoes limited hepatic metabolism.
Side Effects
Contraindications
- Hypersensitivity to cefixime or any other cephalosporin antibiotics.
- History of severe hypersensitivity reaction to penicillin.
Drug Interactions
Nifedipine
May increase the absorption of cefixime.
Carbamazepine
Cefixime may increase carbamazepine plasma levels, necessitating monitoring.
Warfarin and other anticoagulants
Cefixime may increase prothrombin time (INR), requiring careful monitoring and possible dose adjustment of anticoagulants.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Symptomatic and supportive treatment should be given.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. However, there are no adequate and well-controlled studies in pregnant women. Use in pregnancy only if clearly needed. Caution should be exercised when cefixime is administered to a nursing mother as it is excreted in human milk.
