Vilatide 25 mcg Inhalation Capsule is a long-acting beta2-adrenergic agonist (LABA) used for the maintenance treatment of chronic obstructive pulmonary disease (COPD) and asthma. It helps to relax the muscles in the airways, improving breathing.
Uses & Indications
Dosage
Adults
The recommended dosage is 25 mcg inhaled once daily, at the same time each day. Do not use more than once every 24 hours.
Elderly
No dosage adjustment is required for elderly patients.
Renal_impairment
No dosage adjustment is required for patients with renal impairment.
How to Take
Vilatide inhalation capsules are for oral inhalation only and should be administered using the provided inhaler device. Do not swallow the capsules. Follow the instructions for use provided with the inhaler.
Mechanism of Action
Vilatide acts selectively on beta2-adrenergic receptors found in the smooth muscles of the airways, leading to bronchodilation. It stimulates intracellular adenyl cyclase, which catalyzes the conversion of ATP to cyclic-3', 5'-adenosine monophosphate (cyclic AMP). Increased cyclic AMP levels relax bronchial smooth muscle and inhibit the release of mediators from mast cells in the airway.

Side Effects
Contraindications
- Hypersensitivity to Vilatide or any component of the formulation.

Drug Interactions
Beta-blockers
May blunt the pulmonary effect of beta-agonists like Vilatide. Non-cardioselective beta-blockers should generally be avoided. If necessary, cardioselective beta-blockers should be used with caution.
Diuretics (e.g., Thiazide or Loop Diuretics)
ECG changes and/or hypokalemia, which may result from the administration of non-potassium-sparing diuretics, can be acutely worsened by beta-agonists. Use with caution.
Strong CYP3A4 Inhibitors (e.g., Ketoconazole, Ritonavir)
Concomitant use may increase systemic exposure to Vilatide, potentially leading to increased cardiovascular adverse effects. Use with caution or avoid.
Storage

Overdose
Symptoms of overdose include excessive beta-adrenergic stimulation, such as tachycardia, palpitations, tremor, headache, and nervousness. Treatment should be supportive and symptomatic. Consider cardiovascular monitoring in severe cases.
