Description
Overview of the medicine
Abecab 10 mg Tablet contains Amlodipine, a dihydropyridine calcium channel blocker. It is used to treat high blood pressure (hypertension) and chest pain (angina). By lowering blood pressure, it helps prevent strokes, heart attacks, and kidney problems. It also relaxes blood vessels and improves blood supply to the heart.
Uses & Indications
Dosage
Adults
Initial dose is 5 mg once daily; may be increased to a maximum of 10 mg once daily if needed.
Elderly
A starting dose of 2.5 mg once daily may be considered; dose escalation should be done with caution.
Renal_impairment
No dose adjustment is required for patients with renal impairment.
How to Take
Take the tablet orally once daily with or without food. Swallow the tablet whole with water. Do not crush or chew.
Mechanism of Action
Amlodipine inhibits the transmembrane influx of calcium ions into vascular smooth muscle and cardiac muscle. The contractile process of cardiac muscle and vascular smooth muscle is dependent upon the movement of extracellular calcium ions into these cells through specific ion channels. Amlodipine reduces peripheral vascular resistance and blood pressure by acting directly on vascular smooth muscle to cause vasodilation.

Onset
Within 6-12 hours for antihypertensive effect.
Excretion
Excreted primarily via the urine (approximately 60% as inactive metabolites, 10% as unchanged drug) and feces (20-25%).
Half life
30-50 hours, allowing once-daily dosing.
Absorption
Well absorbed after oral administration; peak plasma concentrations achieved 6-12 hours post-dose. Bioavailability is 64-90%.
Metabolism
Extensively metabolized in the liver to inactive metabolites (approximately 90%).
Side Effects
Contraindications
- Known hypersensitivity to Amlodipine or other dihydropyridines.
- Severe hypotension.
- Cardiogenic shock.
- Obstruction of the outflow tract of the left ventricle (e.g., high grade aortic stenosis).
- Unstable angina (except Prinzmetal’s angina).
Drug Interactions
Sildenafil
May cause additive hypotensive effects; monitor blood pressure.
Tacrolimus
Increased tacrolimus blood levels, monitor tacrolimus concentrations and adjust dose.
Simvastatin
Limit simvastatin dose to 20 mg daily when co-administered with Amlodipine.
CYP3A4 Inhibitors (e.g., Ketoconazole, Ritonavir)
Increased exposure to Amlodipine, requiring dose reduction.
CYP3A4 Inducers (e.g., Rifampicin, St. John's Wort)
Decreased plasma concentrations of Amlodipine, potentially reducing efficacy.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include excessive peripheral vasodilation with marked hypotension and possibly reflex tachycardia. Management involves active cardiovascular support, including frequent monitoring of blood pressure and heart rate, and elevation of extremities. If hypotension persists despite adequate fluid administration, vasopressors may be considered. Calcium gluconate may reverse the effects of calcium channel blockade.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not known whether Amlodipine is excreted in human milk. Exercise caution when administering to a nursing mother.
