Description
Overview of the medicine
Aces Plus Tablet is a combination medicine used for the relief of mild to moderate pain and fever. Paracetamol provides pain relief and reduces fever, while caffeine acts as an adjuvant, enhancing the analgesic effect of paracetamol and combating drowsiness.
Uses & Indications
Dosage
Adults
1-2 tablets every 4-6 hours as needed. Do not exceed 8 tablets (4000 mg Paracetamol) in 24 hours.
Elderly
No specific dose adjustment is usually necessary, but caution is advised in elderly patients with impaired renal or hepatic function.
Renal_impairment
Use with caution in severe renal impairment. Dose interval may need to be extended.
How to Take
Aces Plus Tablet should be taken orally with water, with or without food.
Mechanism of Action
Paracetamol is thought to act primarily in the central nervous system, inhibiting prostaglandin synthesis. Caffeine is a central nervous system stimulant, which enhances the analgesic effect of paracetamol by increasing its absorption and acting on adenosine receptors.
Pharmacokinetics
Onset
Analgesic effect usually starts within 30-60 minutes.
Excretion
Mainly excreted in urine as metabolites. A small amount of unchanged paracetamol and caffeine is also excreted.
Half life
Paracetamol: Approximately 1-4 hours. Caffeine: Approximately 3-7 hours.
Absorption
Both paracetamol and caffeine are rapidly and well absorbed from the gastrointestinal tract following oral administration.
Metabolism
Paracetamol: Primarily metabolized in the liver via glucuronidation and sulfation, with a small amount by cytochrome P450 to a toxic intermediate (NAPQI). Caffeine: Extensively metabolized in the liver by cytochrome P450 enzymes (CYP1A2).
Side Effects
Contraindications
- Hypersensitivity to paracetamol, caffeine, or any component of the formulation
- Severe hepatic impairment
- Severe renal impairment (for high doses of paracetamol)
- Severe cardiovascular disease (due to caffeine)
- Anxiety disorders or conditions aggravated by caffeine
Drug Interactions
Alcohol
Increases the risk of hepatotoxicity with paracetamol.
Warfarin
May increase the anticoagulant effect of warfarin with prolonged, high-dose paracetamol use, increasing bleeding risk.
Oral Contraceptives
May decrease caffeine clearance.
CNS Stimulants (e.g., ephedrine, pseudoephedrine)
May enhance stimulant effects due to caffeine.
Certain Antibiotics (e.g., Ciprofloxacin, Erythromycin)
May inhibit caffeine metabolism, leading to increased caffeine levels and side effects.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of paracetamol overdose include nausea, vomiting, abdominal pain, and liver damage (which may be delayed for 24-48 hours). Caffeine overdose can cause nervousness, insomnia, tremors, tachycardia, and arrhythmias. Treatment involves gastric decontamination and administration of N-acetylcysteine for paracetamol toxicity.
Pregnancy & Lactation
Paracetamol is generally considered safe during pregnancy when used at recommended doses. Caffeine should be used in moderation. Consult a doctor before use during pregnancy or lactation. Both components are excreted in breast milk.
