Description
Overview of the medicine
Aclobet-005 Ointment contains Clobetasol Propionate, a very potent topical corticosteroid, used for short-term treatment of inflammatory and pruritic manifestations of steroid-responsive dermatoses.
Uses & Indications
Dosage
Adults
Apply a thin film to the affected skin area once or twice daily. Treatment should be limited to 2-4 weeks. Total dose should not exceed 50g per week.
Elderly
No specific dosage adjustment is generally required, but caution should be exercised due to potential for thinner skin and increased systemic absorption.
Renal_impairment
No specific dosage adjustment is recommended for topical application.
How to Take
For external use only. Apply a thin layer to the affected area and rub in gently. Wash hands thoroughly after application, unless the hands are the area being treated.
Mechanism of Action
Clobetasol Propionate works by inducing phospholipase A2 inhibitory proteins, which are collectively called lipocortins. These proteins control the biosynthesis of potent mediators of inflammation such as prostaglandins and leukotrienes by inhibiting the release of their common precursor, arachidonic acid.
Pharmacokinetics
Onset
Clinical improvement usually observed within a few days of application.
Excretion
Excreted renally.
Half life
Not specifically determined for topical application, but systemically absorbed corticosteroids have a half-life of several hours.
Absorption
Systemic absorption can occur through intact skin, and is increased by occlusive dressings, inflammation, and other skin diseases. Absorption is greater on the face, scrotum, and intertriginous areas.
Metabolism
Primarily metabolized in the liver.
Side Effects
Contraindications
- Hypersensitivity to clobetasol propionate or any component of the formulation.
- Untreated cutaneous infections (bacterial, fungal, viral, parasitic).
- Rosacea, acne vulgaris, perioral dermatitis, pruritus without inflammation.
- Perianal and genital pruritus.
Drug Interactions
Systemic Corticosteroids
Concurrent use may increase the risk of systemic corticosteroid adverse effects.
CYP3A4 Inhibitors (e.g., Ritonavir, Itraconazole)
May inhibit the metabolism of clobetasol, leading to increased systemic exposure and potential for adverse effects.
Storage
Store below 30°C. Protect from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Acute overdose is unlikely due to topical application. However, chronic excessive use or misuse can lead to systemic corticosteroid effects such as adrenal suppression and Cushing's syndrome. Management involves gradual withdrawal under medical supervision.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. Clobetasol is excreted in human milk; use with caution in nursing mothers, avoiding application to the breast area.

Clinical Trials
Clobetasol Propionate has been extensively studied in numerous clinical trials establishing its efficacy and safety for various steroid-responsive dermatoses. Its use is supported by decades of clinical experience.
Lab Monitoring
- Not routinely required for typical short-term topical use. If systemic absorption is suspected (e.g., prolonged extensive use), evaluate for HPA axis suppression via ACTH stimulation test or plasma cortisol levels.
Doctor Notes
- Emphasize the potent nature of clobetasol propionate and the importance of short-term use.
- Advise patients on proper application technique and areas to avoid (e.g., face, intertriginous areas).
- Monitor for signs of skin atrophy, infection, and systemic absorption, especially in children or with extensive use.
Patient Guidelines
- Use only as directed by your doctor and for the prescribed duration, typically not exceeding 2-4 weeks.
- Apply a very thin layer to the affected area only.
- Avoid applying to the face, groin, armpits, or under breasts unless specifically instructed by your doctor.
- Do not use occlusive dressings (bandages, wraps) over the treated area unless advised by your doctor, as this can increase systemic absorption.
- Wash your hands thoroughly after each application.
Missed Dose Advice
If you miss a dose, apply it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not apply a double dose to make up for a missed one.
Driving Precautions
Aclobet-005 Ointment is not known to affect the ability to drive or operate machinery.
Lifestyle Advice
- Maintain good skin hygiene.
- Avoid irritants and allergens that may worsen your skin condition.
