Description
Overview of the medicine
Adflox 500 mg Capsule is an antibiotic belonging to the fluoroquinolone class. It is used to treat a variety of bacterial infections.
Uses & Indications
Dosage
Adults
Typically 250-750 mg twice daily, depending on the infection type and severity. For 500mg capsule, usually 500 mg twice daily.
Elderly
No specific dose adjustment needed based on age alone, but renal function should be considered.
Renal_impairment
Dose adjustment required. For CrCl 30-60 mL/min, 250-500 mg every 12 hours. For CrCl <30 mL/min, 250-500 mg every 18-24 hours.
How to Take
Take with a glass of water, with or without food. Do not take with dairy products or calcium-fortified juices alone. Swallow the capsule whole, do not chew or crush.
Mechanism of Action
Ciprofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, enzymes essential for bacterial DNA replication, transcription, repair, and recombination. This leads to disruption of bacterial DNA and cell death.
Pharmacokinetics

Within 1-2 hours
Excretion
Primarily excreted unchanged in the urine (approximately 50-70%), and a smaller portion in the feces.
Half life
Approximately 3-5 hours
Absorption
Rapidly and well absorbed from the gastrointestinal tract; bioavailability is approximately 70-80%.
Metabolism
Partially metabolized in the liver to four active metabolites.
Side Effects
Contraindications
- Hypersensitivity to ciprofloxacin or other quinolones.
- Concomitant administration with tizanidine.
Drug Interactions
Warfarin
Potentiation of anticoagulant effect, increased bleeding risk.
Tizanidine
Increased risk of hypotension and sedation.
Theophylline
Increased plasma concentrations of theophylline, leading to toxicity.
Dairy products/Calcium-fortified juices
Reduced absorption of ciprofloxacin. Do not take ciprofloxacin with these alone.
Antacids containing aluminum or magnesium
Reduced absorption of ciprofloxacin. Administer ciprofloxacin 2 hours before or 6 hours after antacids.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, diarrhea, headache, dizziness, confusion, and sometimes seizures. Management is primarily supportive and symptomatic, including gastric lavage or emesis. Adequate hydration should be maintained.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. Ciprofloxacin is excreted in breast milk; therefore, it is generally not recommended during breastfeeding due to potential risk of arthropathy in the infant.
