Description
Overview of the medicine
Cefixime is an oral third-generation cephalosporin antibiotic used to treat a wide range of bacterial infections, especially in children, due to its effectiveness against various gram-positive and gram-negative bacteria.
Uses & Indications
Dosage
Adults
400 mg once daily or 200 mg every 12 hours, depending on the infection. Not typically supplied as a 100 mg suspension for adults.
Elderly
No specific dosage adjustment needed for elderly patients unless there is severe renal impairment.
Renal_impairment
Dosage adjustment is necessary. For CrCl 21-60 mL/min: 300 mg daily. For CrCl < 20 mL/min: 200 mg daily.
How to Take
Oral administration. The suspension can be taken with or without food. It is crucial to shake the bottle well before each use to ensure even distribution of the medication. Use the provided measuring device for accurate dosing.
Mechanism of Action
Cefixime inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Rapid, typically within 1-2 hours of administration.
Excretion
Excreted primarily via urine and bile, with significant renal clearance.
Half life
Plasma elimination half-life is typically 3 to 4 hours.
Absorption
About 30-50% of an oral dose is absorbed. Peak plasma concentrations are achieved in 2 to 6 hours.
Metabolism
Minimal hepatic metabolism; primarily excreted as unchanged drug.
Side Effects
Contraindications
- Hypersensitivity to cefixime or any other cephalosporin antibiotics.
- History of severe hypersensitivity (e.g., anaphylaxis) to penicillins.
Drug Interactions
Nifedipine
May increase cefixime absorption.
Carbamazepine
Concomitant administration may increase plasma carbamazepine levels.
Warfarin and other anticoagulants
May increase the anticoagulant effect, leading to increased INR and potential bleeding risk. Close monitoring of INR is recommended.
Storage
Store the dry powder at controlled room temperature (15-30°C), away from moisture and direct light. Once reconstituted, store the suspension in a refrigerator (2-8°C). Do not freeze.
Overdose
In case of overdose, gastric lavage may be indicated. Hemodialysis or peritoneal dialysis are not effective in removing cefixime from the body.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Cefixime is excreted in small amounts in human milk; use with caution during breastfeeding.

Lab Monitoring
- Renal function tests (BUN, creatinine) particularly in patients with pre-existing renal impairment or during prolonged therapy.
- Complete blood count (CBC) if therapy is prolonged.
- INR monitoring if co-administered with anticoagulants like warfarin.
Doctor Notes
- Confirm bacterial susceptibility to cefixime before initiating treatment, especially for severe infections.
- Counsel patients on the importance of completing the full course of therapy to prevent antibiotic resistance.
- Be alert for signs of C. difficile-associated diarrhea (e.g., severe watery diarrhea, abdominal pain, fever) during or after treatment.
Patient Guidelines
- Always complete the full course of antibiotics as prescribed, even if symptoms improve, to prevent resistance and ensure complete eradication of the infection.
- Shake the suspension bottle vigorously before each use.
- Use the provided measuring spoon or cup to ensure accurate dosing, especially for children.
- Store the reconstituted suspension as directed (usually refrigerated) and discard any unused portion after the recommended period.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose to catch up.
Driving Precautions
Cefixime generally does not impair the ability to drive or operate machinery. However, if side effects such as dizziness occur, patients should avoid such activities.
Lifestyle Advice
- Maintain good hygiene to help prevent the spread of infection.
- Ensure adequate hydration during antibiotic therapy.
- Consult your doctor if symptoms do not improve or worsen after a few days of treatment.
