Overview of the medicine
Alagra 120 mg Tablet contains Fexofenadine, a non-sedating antihistamine. It is used to relieve symptoms of seasonal allergic rhinitis (hay fever) and chronic idiopathic urticaria (hives).
Uses & Indications
Dosage
Adults
For seasonal allergic rhinitis: 120 mg once daily or 60 mg twice daily. For chronic idiopathic urticaria: 180 mg once daily.
Elderly
No specific dose adjustment is generally required for elderly patients with normal renal function. Monitor renal function in elderly.
Renal_impairment
For adult patients with impaired renal function (creatinine clearance < 80 mL/min), including those on dialysis: 60 mg once daily. Consult a doctor for specific guidance.
How to Take
Take orally with water, preferably before a meal. Do not take with fruit juices (e.g., grapefruit, orange, apple) as they can reduce absorption. Swallow the tablet whole; do not chew or crush.
Mechanism of Action
Fexofenadine is a selective peripheral H1-receptor antagonist. It blocks the effects of histamine, a natural substance in the body that causes allergy symptoms like sneezing, runny nose, watery eyes, and itching.

Onset of action within 1 hour.
Excretion
Primarily excreted unchanged via bile into feces (approximately 80%) and urine (approximately 11%).
Half life
Plasma elimination half-life of approximately 11-15 hours.

Rapidly absorbed after oral administration; peak plasma concentrations reached in 1-3 hours.
Metabolism
Minimally metabolized; approximately 5% of the total dose is metabolized in the liver.
Side Effects
Contraindications
- Known hypersensitivity to fexofenadine or any of the excipients.
Drug Interactions
Erythromycin and Ketoconazole
Concomitant use with erythromycin or ketoconazole can increase plasma concentrations of fexofenadine, but this is generally not associated with increased adverse effects.
Antacids (aluminum and magnesium containing)
Co-administration with aluminum or magnesium containing antacids should be avoided within 2 hours of fexofenadine, as they can significantly reduce fexofenadine absorption.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, dizziness, and dry mouth. Management is symptomatic and supportive. Hemodialysis does not effectively remove fexofenadine from blood.
Pregnancy & Lactation
Pregnancy Category C. Use in pregnancy only if the potential benefit justifies the potential risk to the fetus. Fexofenadine is excreted into breast milk; caution should be exercised when administered to a nursing mother. Consult your doctor.
