Description
Overview of the medicine
Alvita 100 mg Tablet contains Thiamine Hydrochloride (Vitamin B1), essential for carbohydrate metabolism and nerve function. It is used to prevent and treat thiamine deficiency.
Uses & Indications
Dosage
Adults
For mild deficiency: 10-25 mg daily. For severe deficiency: 50-100 mg daily in divided doses. For Wernicke-Korsakoff: 100 mg orally or parenterally initially, then 50-100 mg daily until symptoms subside.
Elderly
Dosage similar to adults, no specific adjustment needed unless severe renal or hepatic impairment.
Renal_impairment
No specific dosage adjustment required, as thiamine is water-soluble and excreted.
How to Take
Oral. Can be taken with or without food. Follow your doctor's or pharmacist's instructions.
Mechanism of Action
Thiamine is converted into its active form, thiamine pyrophosphate (TPP), which is a coenzyme for several enzymes involved in carbohydrate metabolism, including pyruvate dehydrogenase and alpha-ketoglutarate dehydrogenase. It is also involved in the pentose phosphate pathway.
Pharmacokinetics
Onset
Clinical improvement in deficiency symptoms can be observed within hours to days.
Excretion
Excreted primarily in the urine, both as unmetabolized thiamine and its metabolites.
Half life
Approximately 98 minutes, with a biological half-life of 18 days for total body thiamine.
Absorption
Well absorbed orally, mainly in the duodenum and ileum, by an active transport mechanism at low concentrations and by passive diffusion at high concentrations.
Metabolism
Metabolized in the liver to thiamine pyrophosphate (TPP) and other inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to thiamine or any component of the formulation.
- Not recommended for treatment of megaloblastic anemia unless combined with other appropriate vitamins (e.g., B12).
Drug Interactions
Alcohol
Chronic alcohol consumption increases thiamine requirements and can impair absorption.
Fluorouracil
Can inhibit the phosphorylation of thiamine, leading to thiamine deficiency.
Diuretics (e.g., Furosemide)
May increase urinary excretion of thiamine.
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children.
Overdose
Thiamine is generally non-toxic even at high doses due to its water-soluble nature and rapid excretion. Symptoms of extreme overdose are rare but may include headache, insomnia, irritability, and rapid pulse. Treatment is generally supportive.
Pregnancy & Lactation
Generally considered safe for use in recommended doses during pregnancy and lactation. Thiamine is an essential nutrient. Consult a doctor.
