Initial dose is 5 mg three times daily, gradually increased by 5 mg increments every 3 days to a maximum of 20 mg three times daily (total 60 mg/day). Max recommended dose is 100 mg/day.
Elderly
Lower starting dose (e.g., 5 mg twice daily) and slower titration are recommended due to increased sensitivity and potential for adverse effects.
Renal_impairment
Dose reduction is necessary. Initial dose should be 5 mg once daily, and subsequent doses should be adjusted cautiously based on response and renal function.
