Description
Overview of the medicine
Bacspa 10 mg Tablet contains Baclofen, a muscle relaxant used to treat muscle spasticity arising from various conditions like multiple sclerosis, spinal cord injury, or cerebral palsy. It works by acting on nerve pathways in the spinal cord to reduce muscle spasms and restore muscle function.
Uses & Indications
Dosage
Adults
Initially 5 mg three times daily for 3 days, then increased by 5 mg increments every 3 days. Typical maintenance dose: 30-75 mg/day in divided doses. Max: 80 mg/day.
Elderly
Lower initial doses (e.g., 5 mg once or twice daily) and slower titration recommended due to increased sensitivity and potential for impaired renal function.
Renal_impairment
Dosage should be significantly reduced; consider 5 mg daily or every other day, monitoring renal function closely. Titrate carefully based on response and tolerability.
How to Take
Bacspa 10 mg Tablet should be taken orally, preferably with food or milk to minimize gastrointestinal upset. Do not chew or crush the tablet. It is important not to discontinue the medication abruptly.
Mechanism of Action
Baclofen acts as an agonist at GABAB receptors, particularly at the spinal cord level. It inhibits both monosynaptic and polysynaptic reflexes, reducing the release of excitatory neurotransmitters and hyperpolarizing neurons, leading to decreased skeletal muscle spasticity.
Pharmacokinetics
Onset
Onset of action is typically within 1-2 hours.
Excretion
Primarily excreted by the kidneys (approximately 70-80% as unchanged drug) within 72 hours.
Half life
The elimination half-life ranges from 2.5 to 4 hours.
Absorption
Rapidly and extensively absorbed from the gastrointestinal tract. Peak plasma concentrations are achieved within 2-3 hours.
Metabolism
Only about 5-15% of the dose is metabolized in the liver; the remainder is excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to baclofen or any component of the formulation
- Acute porphyria
Drug Interactions
Antihypertensives
Additive hypotensive effects may occur, requiring dosage adjustment.
Levodopa/Carbidopa
Exacerbation of parkinsonian symptoms has been reported in some patients.
Tricyclic Antidepressants, MAOIs
Enhanced CNS depression and possible potentiation of hypotensive effects.
CNS Depressants (e.g., alcohol, opioids, benzodiazepines)
Increased central nervous system (CNS) depression, including sedation and respiratory depression.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, dizziness, severe muscle hypotonia, respiratory depression, seizures, and coma. Management is supportive, involving maintaining an open airway, cardiovascular support, and hydration. There is no specific antidote for baclofen overdose.
Pregnancy & Lactation
Pregnancy Category C. Use in pregnancy only if the potential benefit justifies the potential risk to the fetus. Baclofen is excreted in breast milk; caution should be exercised when administered to a nursing mother.

Lab Monitoring
- Renal function tests (blood urea nitrogen and creatinine) in patients with impaired renal function or elderly.
- Liver function tests (ALT, AST) periodically for patients on long-term therapy.
Doctor Notes
- Initiate baclofen therapy with a low dose and titrate slowly to optimal effect while minimizing adverse events.
- Emphasize to patients and caregivers the critical importance of gradual dose reduction upon discontinuation to prevent severe withdrawal symptoms.
- Monitor for signs of CNS depression, especially when co-administered with other CNS depressants. Adjust dose in renal impairment.
Patient Guidelines
- Take Bacspa exactly as prescribed by your doctor; do not change the dose or stop abruptly without consulting your doctor.
- This medicine may cause drowsiness or dizziness; avoid activities requiring mental alertness such as driving or operating machinery.
- Inform your doctor about all other medications, herbal products, and supplements you are taking to avoid potential drug interactions.

If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose to catch up.
Driving Precautions
Bacspa can cause drowsiness, dizziness, and decreased mental alertness. Patients should be warned not to drive, operate hazardous machinery, or engage in activities requiring full mental alertness until they know how the drug affects them.
