Uses & Indications
Dosage
Adults
Not typically used for adults in this strength. For adult use, higher strength tablets are preferred (e.g., 500 mg).
Elderly
Not typically used for elderly in this strength. Dosage for elderly with higher strength paracetamol may require adjustment in severe renal impairment.
Renal_impairment
Dose interval may need to be extended in severe renal impairment (CrCl < 10 mL/min) to every 8 hours.
How to Take
Oral administration. Shake the bottle well before use. Use a measuring spoon or oral syringe for accurate dosing. Can be taken with or without food.
Mechanism of Action
Paracetamol is thought to act primarily in the central nervous system, inhibiting prostaglandin synthesis (COX-1 and COX-2) in the brain. It also acts peripherally to block pain impulse generation and has antipyretic effects by acting on the hypothalamic heat-regulating center.

Excretion

Metabolism
Primarily metabolized in the liver via glucuronidation and sulfation, with a small portion metabolized by cytochrome P450 into a toxic intermediate (NAPQI) which is rapidly detoxified by glutathione.

Alcohol
Increases the risk of liver damage with chronic heavy alcohol consumption.
Cholestyramine

Metoclopramide and Domperidone
