Bestcol 20 mg tablet contains Rosuvastatin, a medication used to lower high cholesterol and triglycerides in the blood. It helps prevent heart attacks, strokes, and other cardiovascular problems by reducing the amount of 'bad' cholesterol (LDL) and increasing 'good' cholesterol (HDL).
Uses & Indications
Dosage
Adults
Initial dose is usually 5 mg or 10 mg once daily. Max dose 40 mg once daily. For 20 mg: one tablet once daily.
Elderly
No dose adjustment required for patients >70 years unless other risk factors for myopathy exist. Initial dose 5 mg for patients >70 years.
Renal_impairment
Mild to moderate impairment: No dose adjustment needed. Severe impairment (CrCl <30 mL/min): Initial dose 5 mg, max 10 mg daily.
How to Take
Bestcol tablet can be taken orally, once daily, with or without food. It is recommended to take it at the same time each day, preferably in the evening.
Mechanism of Action
Rosuvastatin works by inhibiting HMG-CoA reductase, an enzyme crucial for cholesterol production in the liver. This reduces the synthesis of cholesterol, leading to a decrease in LDL-cholesterol levels and an increase in HDL-cholesterol.

Onset
Lipid-lowering effects are evident within 1 week, with 90% of maximum response achieved by 2 weeks.
Excretion
Approximately 90% is excreted unchanged in feces, with the remaining 10% excreted in urine.
Half life
Approximately 19 hours.
Absorption
Approximately 20% oral bioavailability. Peak plasma concentrations are reached about 3-5 hours after oral administration.
Metabolism
Limited metabolism, primarily by CYP2C9 to an inactive N-desmethyl metabolite. Main component is unchanged rosuvastatin.
Side Effects
Contraindications
- Hypersensitivity to rosuvastatin or any component of the formulation
- Active liver disease, including unexplained persistent elevations of serum transaminases
- Pregnancy and lactation
- Severe renal impairment (CrCl <30 mL/min) at doses of 40 mg
- Concomitant use with ciclosporin
Drug Interactions
Warfarin
May enhance anticoagulant effect; monitor INR.
Ciclosporin
Significantly increases rosuvastatin exposure; concomitant use is contraindicated.
Gemfibrozil
Increases rosuvastatin exposure; limit rosuvastatin dose to 10 mg daily.
Niacin (high doses)
Increased risk of myopathy.
Protease inhibitors (e.g., Atazanavir/Ritonavir)
Increased rosuvastatin exposure; dose adjustment may be necessary.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
There is no specific treatment for rosuvastatin overdose. In the event of an overdose, symptomatic treatment and supportive measures should be initiated. Liver function tests and CK levels should be monitored.
Pregnancy & Lactation
Rosuvastatin is contraindicated during pregnancy and lactation. Women of childbearing potential should use effective contraception.
