Overview of the medicine
Broklin-CR 825 mg Tablet is an extended-release fluoroquinolone antibiotic used to treat a variety of bacterial infections, including urinary tract infections, respiratory tract infections, and skin infections. The controlled-release formulation allows for once-daily dosing.
Uses & Indications
Dosage
Adults
For uncomplicated urinary tract infections: 825 mg once daily for 3 days. For complicated urinary tract infections: 825 mg once daily for 7-14 days. For chronic bacterial prostatitis: 825 mg once daily for 28 days.
Elderly
No specific dosage adjustment required for elderly patients unless renal function is significantly impaired.
Renal_impairment
For creatinine clearance (CrCl) < 30 mL/min, the dosage should be reduced. Consult product leaflet for specific guidelines.
How to Take
Take orally, with or without food. Swallow the tablet whole; do not crush, break, or chew it.
Mechanism of Action
Ciprofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, enzymes necessary for bacterial DNA replication, transcription, repair, and recombination. This leads to disruption of bacterial DNA and cell death.

Onset
Onset of antibacterial action occurs within hours; clinical improvement seen within days.
Excretion
Primarily excreted unchanged by the kidneys (approximately 50-70% via urine) and via feces (approximately 15-30%).
Half life
Elimination half-life is approximately 4-5 hours, but the controlled-release formulation maintains therapeutic concentrations over 24 hours.
Absorption
Well absorbed orally. Peak plasma concentrations are achieved in approximately 6-8 hours for the extended-release formulation. Absolute bioavailability is about 70%.
Metabolism
Partially metabolized in the liver to several less active metabolites (approximately 10-30% of dose).
Side Effects
Contraindications
- Hypersensitivity to ciprofloxacin or any other fluoroquinolone
- Concomitant administration with tizanidine
Drug Interactions
Warfarin
May enhance the anticoagulant effect of warfarin, requiring INR monitoring.
Tizanidine
Concomitant use is contraindicated due to increased risk of hypotension and sedation.
Theophylline
Increased plasma concentrations of theophylline, leading to potential toxicity.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
Concurrent use may increase the risk of CNS stimulation and convulsive seizures.
Antacids containing magnesium or aluminum, sucralfate, didanosine (chewable/buffered tablets or pediatric powder), or products containing iron or zinc
Reduced absorption of ciprofloxacin. Administer ciprofloxacin at least 2 hours before or 6 hours after these agents.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of acute overdose, gastric emptying (e.g., induced vomiting or gastric lavage) is recommended. Supportive treatment should be initiated, and the patient should be kept well hydrated. Hemodialysis or peritoneal dialysis does not remove ciprofloxacin significantly.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Ciprofloxacin is excreted into breast milk and may cause serious adverse reactions in nursing infants; therefore, a decision should be made whether to discontinue nursing or to discontinue the drug.
